Responsibilities include:</p><ul><li>Screens patients for the eligibility of and assists with completion of needed applications and paperwork for:<ul><li>sliding fee scale/discount fee program</li><li>subsidized health insurance programs</li><li>charity care programs, including local hospital applications</li><li>medication assistance programs</li><li>addressing legal issues</li><li>other programs that assist with social determinants of health (i.e. food insecurity, housing insecurity, transportation insecurity)&nbsp;</li></ul></li><li>Conducts enrollments into programs when possible, and provides referrals to partner agencies, as appropriate.</li><li>Maintains certification as a Certified Application Counselor for the organization and completes all required training(s) to maintain certification.</li><li>Provides Senior&rsquo;s Health Insurance Information Program (SHIIP) counseling to Medicare beneficiaries, as requested.</li><li>Educates patients on health center advocacy and voter registration and provides related assistance.</li></ul><p data-pasted=\"true\">What We&#39;re Looking For:</p><ul data-pasted=\"true\"><li>Bachelor&rsquo;s degree in healthcare or social services related field, preferred.
