They are not intended as an exhaustive list of all job duties, responsibilities and requirements.)</p>\n<ol>\n <li>Performs patient evaluations to determine patient deficits and need for therapy.</li>\n <li>Develops an individualized patient plan of care, physical therapy interventions and expected outcomes based on the evaluation and documents findings.</li>\n <li>Implements or supervises the patient treatment within the established plan of care.</li>\n <li>Evaluates or reassesses patient progress towards established goals.</li>\n <li>Supervises Physical Therapist Assistants and Techs on rotation.</li>\n <li>Assists in discharge planning including communication to the patient, family and other health care professionals.</li>\n <li>Exercises the skills, safeguards and professional conduct of a Physical Therapist.</li>\n <li>Completes other duties and special projects as assigned.</li>\n <li>Participates and reports in weekly patient conference with current patient progress and discharge planning with team staff members.</li>\n <li>Performs home assessments as coordinated and assigned by the therapy supervisor.</li>\n</ol>\n<p><strong>Secondary Job Duties That May be Reassigned:</strong></p>\n<ol>\n <li>Responsible for reporting breakdown of necessary support services to supervisor.</li>\n <li>Assist in orienting staff members.</li>\n <li>Supervise PT /PTA clinical students, teen volunteers and shadow students.</li>\n <li>Assist with departmental Performance Improvement Program and maintain FIM certification.</li>\n <li>Maintain adequate supplies, assist in keeping department clean and ready for patient treatment.</li>\n</ol>",
