The Role
Under general direction, The Peer Professional is responsible for providing community-based recovery services to PARC Bronx/Queens program and to Community Oriented Rehabilitation and Empowerment Services (CORES) participants. He/she is responsible for conducting outreach, advocacy, and developing sober social and support activities while documenting all provided services in a manner that is compliant with regulatory and funding agents. He/she delivers services to the participants on-site and in the community. This work is carried out in support of the mission and goals of Samaritan Daytop Village.
Responsibilities
What You Will Do
Provide CORE services to participants on your caseload including Community Psychiatric Support and Treatment (CPST), Psychosocial Rehabilitation (PSR), Family Support and Training (FST) and Empowerment Services – Peer Support via telehealth, in the client’s home and/or in the community.
Produce and maintain thorough, accurate, and timely documentation, including documentation of interactions with participants, services provided, important information and/or events, and contacts with other agencies and service providers, as required by agency policies, and relevant contracts and regulatory agencies.
Facilitate referrals to Recovery Oriented Systems of Care (ROSC) providers within the community, working collaboratively to support the participant in accessing services, advocating for individual needs and any changes to Plan of Care, if applicable.
Support PARC Centers and CORES participants in developing and implementing a person-centered recovery plan that is relevant to the current expressed needs of the participant.
Meet with each participant on a regular basis and provide support in line with the goals outlined in the recovery plan via telehealth or in-person sessions to promote choice and supported risk in achieving recovery goals.
Provide companionship and accompaniment to Participants during community appointments, recovery-oriented activities, and during stressful transitions such as pre- and post-admission to emergency rooms, detox units, and discharges from hospitals and jails.
Implement community outreach plan; conduct community presentations to attract new partnerships, volunteers, and participants.
Provide Naloxone (Narcan) training and HARM Reduction education to PARC and CORES participants, community members and providers.
Complete Government Performance & Results Act (GPRA) intake, six months follow ups and Surveys on eligible participants.
Qualifications
Who You Will Be
Certified Recovery Peer Advocate (CRPA) or CRPA-Provisional Certificate.
High School Diploma or GED.
Computer literacy including familiarity in Microsoft Office Suite and EHR.
Knowledge or shared lived experience with OUD and/or StUD
Willingness to travel throughout the required catchment area as needed for applicable programs and able to navigate the public transportation systems to client’s home and within the community.
Must have ability to work a flexible schedule. This position follows non-standard hours including some Saturday and evening work.
Knowledge and commitment to recovery-oriented systems of care.
Demonstrate ability to work independently.
Excellent time management with the ability to meet deadlines.
Ability to think logically, prioritize and follow procedures and instructions.
Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
Experience with advocacy, outreach and engagement, self-help, recovery and transitional supports.
