We w</span><span style=\"color: rgb(48, 48, 48); font-family: tahoma, sans-serif;\">ork as an extension of the hospital consortiums you trust in your community, with the goal of improving care across the continuum for our homebound patients.&nbsp;</span><span style=\"background-color: transparent; font-family: tahoma, sans-serif;\">&nbsp;</span><span style=\"background-color: transparent; font-family: tahoma, sans-serif;\">Join our team and make a difference in your community as we help patients in Batesville and the surrounding areas.&nbsp;</span></p><p><br></p><div><span style=\"font-size: 14px; font-family: tahoma, sans-serif;\"><strong>Job Description</strong></span><p><span style=\"font-size: 14px; font-family: tahoma, sans-serif;\">The Home Health Aide/CNA provides direct patient care to adult and geriatric patients in the home setting.&nbsp;</span></p><ul><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Provides personal care activities of daily living such as bathing, shampooing, oral hygiene, skin care/nail care, shaving, and dressing.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Communicates changes in patient status to the supervising Registered Nurse (RN) in a timely manner.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Reports patient complaints to the RN</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Creates and submits legible, accurate and timely documentation. &nbsp;</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Performs duties in compliance with agency policies and procedures and local, state, and federal regulations.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Demonstrates attention to details for patient&#39;s safety and safety of their home environment.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Reports unsafe conditions in the home. &nbsp;</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Practices effective infection control and adheres to infection control policies and procedures.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Practices safe lifting and transferring of patients. &nbsp;&nbsp;</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Communicates clearly and positively with patients, families, team members and others.&nbsp;</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Displays a caring, patient&rsquo;s first behavior. &nbsp;</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Keeps patients and/or family informed regarding visit time and schedule changes.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Attends in-service programs to meet compliance requirements</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Maintains patient confidentiality in compliance with HIPAA</li></ul><p><span style=\"font-size: 14px; font-family: tahoma, sans-serif;\"><strong>Basic Qualifications</strong></span></p><ul><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Must be a Certified Nursing Assistant (CNA) and/or possess current Home Health Aide registrar in the state of Indiana.&nbsp;</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Must have high school diploma or equivalent</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Must possess valid driver&rsquo;s license and automobile insurance in the state of Indiana</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Must be able to provide own transportation to assignments.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Ability to work independently</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Ability to work in and support a team environment. &nbsp;</li></ul><div><span style=\"font-size: 14px; font-family: tahoma, sans-serif;\">Suburban Home Health is an Equal Opportunity Employer and prohibits discrimination and harassment of any kind.
