If you're passionate about making a difference when it matters most, we invite you to join our dedicated hospice care team.</p>\n<h2>Essential Functions</h2>\n<ul>\n <li>Provide direct personal care to terminally ill patients</li>\n <li>Assist with personal hygiene, grooming, and toileting</li>\n <li>Support safe mobility, including use of walkers and wheelchairs</li>\n <li>Help with meal preparation, feeding, and light housekeeping</li>\n <li>Observe and report changes in the patient’s physical or mental condition</li>\n <li>Offer emotional and psychological support to patients and families</li>\n <li>Maintain accurate and timely documentation of visits</li>\n <li>Attend team meetings and participate in Quality Improvement (QI) activities</li>\n <li>Practice<strong> </strong>infection control and follow safety protocols at all times</li>\n</ul>\n<h2>About You</h2>\n<p><strong>Qualifications – What You’ll Bring:</strong></p>\n<ul>\n <li>Active CHHA license in the state of employment (Required)</li>\n <li>Current CPR certification (Required)</li>\n <li>Driver’s license and reliable vehicle with insurance (Required)</li>\n <li>Six (6) months of direct patient care experience (Required)</li>\n <li>Comfortable working in home settings, hospice facilities, or long-term care environments</li>\n <li>Demonstrated compassion, patience, and ability to handle emotional situations</li>\n <li>Willingness to work independently and adapt to changing patient needs</li>\n</ul>\n<p> </p>\n<p><strong>Preferred Background (Not Required):</strong></p>\n<ul>\n <li>Experience in home health, hospice, palliative care, or oncology</li>\n <li>Prior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homes</li>\n <li>Exposure to end-of-life care, wound care, or admissions support</li>\n</ul>\n<p> </p>\n<p>Compensation may vary within the salary range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity.
