Join us in making a real difference in the health of our community.</em></p>\n<h2>Job Summary</h2>\n<p>We are seeking an operations manager to join our ACO team to oversee day oversee the day-to-day operational functions of our Provider Accountable Care Organization (ACO). This individual will play a critical role in coordinating cross-functional activities that drive performance improvement, ensure compliance with ACO quality and cost metrics, and support provider engagement. The ideal candidate has a strong background in healthcare operations, project management, and value-based care delivery models.</p>\n<h2>Responsibilities</h2>\n<p><strong>1. Project management:</strong></p>\n<p> </p>\n<p>a. Lead the planning, implementation and tracking of value-based care initiatives in daily operations</p>\n<p>b. Develop and maintain project timelines, deliverables and achieve KPIs aligned with organizational goals.</p>\n<p>c. Coordinate with cross-functional teams to achieve project objectives.</p>\n<p> </p>\n<p><strong>2. Planning and strategy execution:</strong></p>\n<p> </p>\n<p>a. Develop and implement tactics to achieve strategic goals (e.g., reduce ED utilization and improve patient engagement)</p>\n<p>b. Develop and implement strategies for engagement and continuity of care for the aligned population to model.</p>\n<p>c. Develop risk stratification models and prioritize care interventions based on need and impact.</p>\n<p>d. Collaborate with leadership to align clinical practices with quality metrics and reimbursement models.</p>\n<p> </p>\n<p><strong>3. Clinical operations support:</strong></p>\n<p> </p>\n<p>a. Develop and implement workflows to support team based and patient centered care.</p>\n<p>b. Integrate roles and responsibilities of each team member in successfully driving the tasks in order to achieve goals.</p>\n<p>c. Identify care gaps and support clinical team in addressing operational issues.</p>\n<p>d. Be a bridge between Clinical staff, leadership and patients by regular meetings to achieve high quality and smooth daily operation.</p>\n<p>e. Coordinate with community partnerships to address social determinants of health and behavioral health in the aligned population</p>\n<p> </p>\n<p><strong>4. Data and Reporting:</strong></p>\n<p> </p>\n<p>a. Develop innovative approach to create reporting along with working with data analytics team to track and report key performance indicators on daily operation.</p>\n<p>b. Evaluate and implement data insights to drive continuous quality operation.</p>\n<p>c. Skilled in using MS-Office (PowerPoint, excel, etc.) for reporting and analysis for daily operation</p>\n<p>d. Maintain records of goals, timelines, outcomes and learnings.</p>\n<p> </p>\n<p><strong>5. Compliance & Reporting:</strong></p>\n<p> </p>\n<p>a. Ensure compliance with CMS regulations, including timely submissions of required documentation and data.</p>\n<p>b. Support audit readiness and maintain operational documentation and protocols.</p>\n<p>6. Technology Integration:</p>\n<p>a. Work with IT teams to enhance EHR use and develop user friendly functions for clinical team.</p>\n<p>b.
