Position Description:
Title: Housing Navigator Reports To: Program Manager
Hours: Full Time Supervises: None
FLSA Status: Non-Exempt (Hourly) OSHA Exposure Category: II
Position Summary:
This position provides housing navigation and paraprofessional case management services to individuals and families experiencing homelessness. The Housing Navigator supports participants in obtaining and maintaining permanent housing through care coordination, housing search assistance, landlord engagement, and direct service support. The Housing Navigator works closely with case managers, service providers working with chronically homeless, at risk of homelessness, and veterans, community partners, and housing providers to ensure participants successfully progress from engagement to housing stability.
Essential Job Functions:
• Serves as liaison between the organization and community partners, including but not limited to veteran service providers, Coordinated Entry, landlords, and service agencies;
• Integrates into multidisciplinary teams, including veteran service systems, to coordinate care and prevent duplication of services;
• Develops and implements individualized Housing Navigation Plans with participants;
• Conducts landlord outreach, recruitment, and relationship building to expand available housing opportunities;
• Identifies and maintains a portfolio of available housing units in the community;
• Assists participants in gathering documentation necessary for housing applications and placement;
• Accompanies participants during housing search, unit viewings, and lease signings;
• Provides paraprofessional case management support to augment services provided by case managers;
• Assists participants in completing forms related to housing, benefits, and supportive services;
• Obtains medical, social, and housing-related records to support service planning;
• Conducts collateral contacts with community agencies such as veteran services, healthcare providers, law enforcement, and social service agencies;
• Accompanies case managers on home visits, emergency visits, and other field-based services;
• Transports participants to housing appointments, service providers, and other essential activities;
• Assists with move-in coordination and supports initial housing stabilization efforts;
• Provides training and education to participants on maintaining housing stability, including tenant responsibilities, lease compliance, and basic household management, or connects participants to appropriate resources to receive such training;
• Gathers and distributes basic needs items such as food, clothing, and household goods;
• Maintains accurate and timely documentation of participant interactions and housing progress;
• Assists with data collection, reporting, filing, and case record maintenance;
• Embraces and embodies the mission, vision, guiding principles, clinical vision, and goals of the organization; and
• Performs all other duties as assigned.
Qualifications
Essential Qualifications:
Education:
• Graduation from an accredited four-year college or university with a degree in Social Work, Psychology, Human Services, Criminal Justice, or a related field is required.
Experience and Competencies:
• Experience working with individuals experiencing homelessness, substance use, or behavioral health challenges is preferred;
• Experience with housing navigation, Coordinated Entry, or working with veteran service systems is preferred;
• Intake and assessment experience is preferred;
• Knowledge of the criminal justice system is preferred;
• Excellent communication skills;
• Ability to exercise good judgment and discretion;
• Ability to work well in a team environment;
• Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality;
• Professional appearance and demeanor;
• Must be culturally/linguistically sensitive to populations served;
• Valid driver’s license and ability to obtain and maintain agency vehicle insurance is required; and
• CPR Certification and First Aid Certification (or ability to obtain).
