We also provide attractive working conditions and opportunities for career growth through professional development.</p>\n<p>Discover why Cooper University Health Care is the employer of choice in South Jersey.</p>\n<h2>Short Description</h2>\n<p>Provides psychosocial assessments, crisis intervention, resources referrals, to facilitate discharge plans, and/or adjustment to illness, and complex discharge planning for patients and their families.</p>\n<p> </p>\n<p>    </p>\n<p>Formulates the discharge plan with patient, families and the care team based upon a needs assessment.</p>\n<p> </p>\n<p>Coordinates appropriate referrals to home care agencies, skilled nursing and rehabilitation centers, and community-based programs.</p>\n<p> </p>\n<p>Coordinates care authorization process with insurers</p>\n<p> </p>\n<p>Coordinates specific details of patient’s hospitalization with Utilization Management department to ensure appropriate admission status</p>\n<h2>Experience Required</h2>\n<p>3 - 5 years health care experience preferred</p>\n<h2>Education Requirements</h2>\n<p>Requires a master's degree in social work (MSW)/ or BSW or a licensed RN, BSN preferred</p>\n<h2>License/Certification Requirements</h2>\n<p>Current NJ SW License, LCSW Preferred.</p>\n<p>or</p>\n<p>Current NJ RN License</p>\n<p> </p>\n<p>ACM (American Case Management Association) preferred</p>\n<p>CCMC (Commission for Case Manager Certification) preferred</p>\n<h2>Salary Min ($)</h2>USD $34.00\n<h2>Salary Max ($)</h2>USD $56.00",
