          "text": "<p>Clinical Nurse Manager - RN - Home Health<br /></p><p>As a Clinical Manager at Nurse On Call by Brookdale, providing skilled nursing, rehabilitation and other home healthcare services since 1989, you shape our patients with the highest possible level of home health nursing care. You tailor that care to each Nurse On Call (a Brookdale Company) patient&apos;s needs, while directly consulting patients, patients&apos; families, the care team and physicians on client-centered solutions. Empathy and compassion mold the core practices of care you generate. You meticulously record patient care and evaluate its progress and effectiveness with a keen attention to detail in each visit. Because your nature is personable, you build trusting relationships in ongoing care visits. As a Clinical Services Manager, you also guide LPN&apos;s and CNA&apos;s in a collaborative effort to deliver exceptional home health care for each Nurse On Call patient.<br /></p>",

          "text": "<p>Your responsibilites:</p><p> \n* Supervises, monitors, and ensures competencies and performance of an assigned home health care team and assuring the delivery of quality home health services. \n* Provides ongoing case management of all patients assuring an accurate evaluation and treatment plan, maintaining our quality model, and utilizing services appropriately to achieve optimal outcomes. \n* Provides clinical oversight for home health patients within assigned team. \n* Reviews electronically submitted documentation and provides feedback to clinicians regarding deficiencies or OASIS corrections to ensure the medical records reflect compliance with medical necessity, homebound status, visit utilization supported by individual patient assessment/ documentation support and transition (discharge) planning. Completes submitted electronic discharge OASIS documentation review within agency defined time frame. \n* Reviews and approves final Plan of Care (485) within Electronic Medical Records (EMR) system. \n* Reviews agency assigned team outcome data, participates in and facilitates improvement activities for agency outcomes performance. \n* Review patient schedules to ensure appropriate coordination of care and clinical needs. \n* Works directly with the assigned home health care team with the ongoing implementation of approved work methods, patient care model, and procedures that reflect elements essential to rendering high quality care.\nRequried skills and </p><p>qualifications:</p><p> \n* 2 years of experience within a licensed and certified (Medicare) home health agency \n* Competent in Federal (Medicare) and Local standards including Conditions of Participation and Local Coverage Determinations \n* Basic level computer skills including Outlook, Word and Excel \n* Valid RN license within practice state of practice \n\n<br /></p><p><br /></p><p><br /></p>",
