We are currently seeking a <strong>Registered Nurse (RN) to join our Renal Transplant Clinic</strong>—a specialized unit dedicated to delivering exceptional care to patients with complex renal needs. </p>\n<p>At PHC, you’ll work in a collaborative, interdisciplinary environment that values innovation, inclusion, and lifelong learning. Be part of our journey as we prepare to open the new St. Paul’s Hospital—a state-of-the-art health campus set to transform care delivery in British Columbia. </p>\n<p><strong>Your day-to-day: </strong> </p>\n<ul>\n <li>Conduct thorough patient/resident assessments through chart reviews, observations, interviews, and collaboration with peers  </li>\n</ul>\n<ul>\n <li>Develop, implement, and evaluate nursing care plans based on evidence-based practices, prioritizing patient/resident needs  </li>\n</ul>\n<ul>\n <li>Adhere to established policies, procedures, and PHC standards while providing high-quality nursing care  </li>\n</ul>\n<ul>\n <li>Monitor patient/resident progress, evaluate outcomes, and adjust care plans as needed for optimal results  </li>\n</ul>\n<ul>\n <li>Maintain accurate records of assessments, care plans, patient/resident status, and discharge plans using manual and computerized systems  </li>\n</ul>\n<ul>\n <li>Identify and address patient/resident and family learning needs, providing education and referrals as necessary  </li>\n</ul>\n<ul>\n <li>Provide direction to staff, promote teamwork, and share clinical knowledge to enhance patient care  </li>\n</ul>\n<ul>\n <li>Support nursing research initiatives and contribute to the improvement of patient/resident care policies and procedures  </li>\n</ul>\n<ul>\n <li>Advocate for patients/residents' rights and dignity, facilitate discharge plans, and collaborate with families and community resources  </li>\n</ul>\n<ul>\n <li>Perform additional duties as assigned to support the healthcare team. </li>\n</ul>\n<h2>Minimum Nurse-to-Patient Ratio</h2>Minimum Nurse-to-Patient Ratios (mNPR) set the minimum number of nurses required to safely care for patients on a given unit. Developed in partnership with the Ministry of Health, the BC Nurses' Union (BCNU), and health organizations across British Columbia, mNPR provides a consistent and transparent approach to staffing that supports safe nursing practice and high-quality patient care.
