    "description": "<p><span style=\"font-weight: bold\">Location:</span> Online (must be Texas resident)</p>\n<p><span style=\"font-weight: bold\">Reports To:</span> Collaborative Care Program Manager</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Role Summary:</span></p>\n<p>The Behavioral Health Care Manager plays a key role in delivering integrated mental health</p>\n<p>services within the Behavioral Health Integration (BHI) and Collaborative Care Model (CoCM)</p>\n<p>frameworks. The Care Manager works in partnership with primary care and behavioral health</p>\n<p>providers to support patients experiencing mental health concerns, particularly those with</p>\n<p>chronic illness or comorbid medical conditions. The Care Manager ensures patients receive</p>\n<p>coordinated, measurement-based, and team-driven care, while also meeting documentation and</p>\n<p>billing requirements for CoCM or BHI services.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Primary Responsibilities:</span></p>\n<ul>\n<li>Serve as the central point of contact for patients enrolled in the BHI or CoCM program.</li>\n<li>Conduct routine behavioral health screenings (e.g., PHQ-9, GAD-7) and track patient progress over time.</li>\n<li>Provide patient outreach, psychoeducation, and support between clinical visits.</li>\n<li>Collaborate with the psychiatric consultant and primary care provider (PCP) to review patient progress and adjust care plans accordingly.</li>\n<li>Document all patient interactions, screenings, care plan updates, and psychiatric recommendations in the medical record.</li>\n<li>Monitor patient adherence to behavioral health treatment plans and assist with follow-up care coordination.</li>\n<li>Ensure patient consent for collaborative care is obtained and documented prior to billing.</li>\n<li>Assist patients in overcoming barriers to care (e.g., scheduling, transportation, understanding treatment).</li>\n<li>Coordinate referrals to specialty mental health providers as needed.</li>\n<li>Support billing processes by maintaining compliance with time tracking, service documentation, and care plan updates as required by CMS and commercial insurers.</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Qualifications:</span></p>\n<ul>\n<li>Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, Nursing, Public Health, or a related field.</li>\n<li>Prior experience in behavioral health, care coordination, or chronic disease management preferred.</li>\n<li>Familiarity with behavioral health screening tools (PHQ-9, GAD-7, etc.).</li>\n<li>Understanding of the Collaborative Care Model and/or BHI billing requirements is highly desirable.</li>\n<li>Strong communication and interpersonal skills to work with patients, families, and multidisciplinary teams.</li>\n<li>Proficiency in electronic health record (EHR) documentation and case management workflows.</li>\n<li>Bilingual abilities are a plus.</li>\n</ul>",
