We are recruiting for the following position:</strong></p><p style=\"text-align:center;\"><span style=\"font-size:11pt;\"><span style=\"font-family:Calibri, sans-serif;\"><b><span style=\"font-size:13pt;\"><span style=\"font-family:'Times New Roman', serif;\"><span style=\"color:#4f81bd;\">Sea Mar is a mandatory COVID-19 and flu vaccine organization</span></span></span></b></span></span></p><p><u>Community Health Worker for MSS -&#160;Posting #26962</u></p><p>Hourly Rate: $21.75</p><p><strong>Position Summary:</strong></p><p>Full-time Community Health Worker position available for the Maternity Support Services (MSS) Program in Seattle, WA. This position requires flexibility, self-initiative, and the ability to manage time, resources, and client caseload efficiently. Applicant must have access to an insured vehicle, and be able to make home visits. Computer experience needed and database entry skills preferred. Under the direction of the program manager, works closely with all MSS team members scheduling appointments, maintaining client&#8217;s charts, enrolling clients into the program, dealing with phone calls and questions about the program. May follow up on clients in their homes and deliver basic health messages, and ensure that utilization of medical care and other community resources are met.&#160;<br><br><strong>Duties and Responsibilities:</strong></p><ul><li>Teaches basic principles of self-care including appropriate utilization of available pre- and post-natal care, thus reducing negative birth outcomes.&#160;</li><li>Connect clients to Sea Mar and non-Sea Mar resources as appropriate and track all resources<br>available to clients. These services may include but are not limited to insurance enrollment, preventive health services, behavioral health, dental and care management.</li><li>Provide appropriate provider and/or resource referrals to the client to help promote well-being<br>within the family.</li><li>Enrolls clients in the First Steps program and enters client information into database.</li><li>Monitor and document clients&#8217; ability to keep regularly scheduled medical and social service<br>appointments, thus ensuring greater client use of appropriate services.</li></ul><p><strong>Qualifications and/or Education:</strong></p><ul><li>High School diploma or GED required.</li><li>One year of health and/or social services experience working with the childbearing family required.</li><li>This position must obtain Basic Life Support (BLS) CPR within 90 days of hire date and is required to maintain current BLS CPR throughout employment.</li><li>Bilingual in English/Spanish is preferred, but not required.
