As a Physical Therapist, you will be responsible for providing and coordinating comprehensive care to patients of all ages through the proper utilization of evaluation, planning and implementation of a treatment plan in accordance with established standards, policies, procedures, protocols and guidelines. </p>\n<p> </p>\n<p>Bachelor’s degree in Physical Therapy.  Must be Licensed or license pending in the State of Illinois and without previous disciplinary action in Illinois or any other state.  Current CPR card.</p>\n<p> </p>\n<p>Schedule</p>\n<p>PRN</p>\n<p>Contact me if interested at [email protected] or schedule a call with me to discuss at</p>\n<p>  https://MemorialHealthIL.as.me/KariWatson</p>\n<p> </p>\n<p> </p>\n<p><strong>Duties:</strong></p>\n<p> </p>\n<li>Maintains flexibility, organizes and prioritizes time to accommodate assigned patient load.</li>\n<li>Meets departmental standards and insurance/reimbursement guidelines for documentation of care.</li>\n<li>Daily notes</li>\n<li>Communicates with doctors</li>\n<li>Recertification’s</li>\n<li>Evaluations</li>\n<li>Billing</li>\n<li>Outcome measures</li>\n<li>Evaluates patients; plans and delivers care and treatment to progress patient to goals agreed upon by patient, family and Physician.<li>Re-evaluates and plans programs on an ongoing basis.</li><li>Competently applies all Physical Therapy modalities.</li><li>Instructs patients and families in home programs.</li><li>Suggests changes in patients’ clinical approaches, and secures orders as appropriate.</li><li>Communicates and collaborates with rehabilitation team.</li><li>Maintains existing equipment and recommends purchase of new items as consistent with Hospital and Department Strategic Plan.</li></li>\n<li>Maintains current professional knowledge.</li>\n<li>Attends all mandatory educational programs and continuous education required for licensure.</li>\n<li>Seeks continuing education to improve quality of care.</li>\n<li>Maintains fiscal responsibility of the Department and the Hospital.
