Job Title: Mobile Case Manager – Shelter, Onsite
Sign on Bonus : Sign on Bonus of $600 (Half paid out at 3 months and the other half at 12 months).
Salary Range : $24.04 – 26.44/hour; $50,000 - $55,000/annually

180 Degrees’ youth shelters serve adolescents ages 12–19 who are seeking safety, stability, and support during critical moments in their lives. Youth may enter shelter through county placements, or because they are experiencing homelessness, running away from home, family conflict, exploitation, or safety concerns. Many youth have experienced trauma, instability, or disrupted relationships and come to us at a time when trust, consistency, and dignity matter deeply.
The Mobile Case Manager (MCM) provides prevention-focused, community-based support that bridges shelter services and the broader community. This role ensures continuity of care for youth exiting shelter through structured aftercare services while maintaining a consistent outreach presence in spaces where youth naturally gather.
As part of 180 Degrees’ continuity-of-care model, the MCM supports youth across transitions and collaborates closely with shelter staff and other Mobile Case Managers across the organization to maintain alignment as youth move between programs and communities. While the position operates with a high level of independence in the field, it remains grounded in supervision, teamwork, and consistent connection to shelter operations.
This position does not have direct reports. This position is 40 hours per week, onsite.
Program Portfolio
Rochester Youth Shelter (Southeast Region)
Rochester Youth Shelter & Community Resource Center: 12-bed shelter serving youth ages 10-19, offering emergency shelter, basic needs support, and on-site services. Foster Care Program (Olmsted County): A foster care service line providing coordinated placement support, compliance oversight, and ongoing care for children and youth ages 0-19 in Olmsted County. Community Outreach Team: Outreach and youth engagement services within Olmsted County and surrounding areas for ages 10-24. Counties Served: Blue Earth, Brown, Dodge, Faribault, Fillmore, Freeborn, Goodhue, Houston, LeSueur, Martin, Mower, Nicollet, Olmsted, Rice, Sibley, Steele, Wabasha, Waseca, Watonwan, Winona. Duties and Responsibilities include the following. Other duties may be assigned.
Aftercare & Continuity of Services
Provide individualized aftercare support for youth exiting shelter for approximately 3–6 months, based on youth need and choice. Offer aftercare services to all self-referred youth and engage additional youth as appropriate and desired. Maintain consistent engagement through in-person meetings, virtual communication, and approved digital platforms, including agency applications. Support youth in achieving stability related to housing, education, employment, health, relationships, and life skills. Collaborate with shelter staff prior to discharge to ensure smooth transitions and continuity of relationships. Work in partnership with other Mobile Case Managers across 180 Degrees to maintain continuity of care, share relevant information, and support youth who move between regions or programs. Community Outreach & Engagement
Conduct proactive outreach in community locations where youth congregate (e.g., schools, community spaces, events). Develop and maintain a trusted, visible presence so youth recognize the MCM as a consistent and safe point of contact. Distribute outreach materials, maintain face-to-face engagement, and contribute to appropriate online outreach strategies. Utilize agency technology and applications to connect youth to services and resources. Participate in internal MCM coordination meetings or communication channels to ensure alignment of outreach strategies and messaging across programs. Shelter Integration & Transition Support
Spend a portion of time onsite at the shelter or main office to maintain strong connection with staff and youth preparing to transition. Participate in transition planning meetings and support youth engagement prior to discharge. Promote continuity between shelter services and community-based support. Documentation & Data Integrity
Accurately and timely document all youth contacts, outreach efforts, and case management activities in the agency database. Maintain strengths-based, clear documentation aligned with organizational expectations and licensing standards. Use data to inform outreach planning and monitor youth engagement. Professional Practice & Accountability
Maintain regular supervision and scheduled onsite presence despite the independent nature of the role. Follow trauma-informed, strengths-based, and culturally responsive practices. Maintain professional boundaries and sound judgment while working independently in the community. Collaborate with internal teams and external partners to support youth stability and prevention efforts. Other duties as assigned.

Minimum age: 21. Bachelor’s degree in human services, psychology, social services, social work, or a related field and two years of related case management experience; case management experience may be substituted in lieu of education on a year-for-year basis. Experience in the nonprofit sector preferred. Valid driver’s license and insurance; dependable transportation for traveling to community partners and events. Ability to clear a Department of Human Services background screening. Essential Competencies:
Strong foundation in trauma-responsive and youth-centered practice. Ability to build rapport with youth and families while maintaining professional boundaries. Skilled in care coordination, advocacy, and collaborative problem-solving. Understanding of confidentiality, data practices, and ethical responsibilities.
