Learn more about us at stellusrx.com.<br></span></p>\n<p><br></p>\n<p><span style=\"font-size: 16px\">ACCOUNTABILITIES<br>• The PRN, Population Health Pharmacist plays a pivotal role in developing, implementing, and overseeing strategies <br>aimed at improving healthcare outcomes for a defined population. This role involves leveraging pharmaceutical expertise, data<br>analysis, and managerial skills to optimize medication therapies, enhance patient adherence, and collaborate with <br>multidisciplinary teams to improve overall health outcomes.<br></span></p>\n<p><br></p>\n<p><span style=\"font-size: 16px\">ROLE AND RESPONSIBILITIES:<br>• Develop, implement, and evaluate population health initiatives centered around medication management, adherence, and <br>optimizing therapeutic outcomes.<br>• Utilize health data analytics to identify trends, gaps in care, and opportunities for intervention to improve medication-related <br>outcomes for the target population.<br>• Collaborate with healthcare providers to ensure appropriate medication selection, dosing, and monitoring, considering <br>population-specific needs and evidence-based guidelines.<br>• Design and implement programs to educate patients about their medications, ensuring understanding, adherence, and <br>management of potential side effects.<br>• Work closely with healthcare teams, including physicians, nurses, social workers, and other healthcare professionals, to <br>coordinate care plans and interventions for improved patient outcomes.<br>• Participate in quality improvement projects aimed at enhancing medication-related outcomes, reducing adverse events, and <br>promoting evidence-based practices.<br>• Stay updated with pharmaceutical regulations, ensuring compliance with relevant laws, policies, and guidelines.<br>• Provide leadership within the pharmacy team, mentorship to junior staff, and contribute to departmental goals and initiatives.<br>MINIMUM QUALIFICATIONS AND REQUIREMENTS:<br>• Doctor of Pharmacy (PharmD) degree from an accredited institution.<br>• Active pharmacist licensure in the state or region of practice. Must also have an active pharmacist license in TX</span></p>\n<p><span style=\"font-size: 16px\">• Advanced training or certification in population health management or a related field (preferred).</span></p>\n<p><span style=\"font-size: 16px\">• Strong analytical and data interpretation skills.<br>• Experience in pharmacy practice, preferably in a population health or managed care setting.<br>• Excellent communication, collaboration, and leadership abilities.<br>• Knowledge of healthcare regulations, policies, and quality improvement methodologies.</span></p>\n<p><span style=\"font-size: 16px\">• Fluent in Spanish (preferred).</span></p>\n<p><span style=\"font-size: 16px\">  </span></p>",
