Description
SLCS is currently seeking a full-time Housing Stabilization Worker for the HART Hub. The HART Hub (Homeless and Addiction Recovery Treatment Hub) supports individuals who are experiencing or at risk of experiencing homelessness and who face challenges related to substance use and/or mental health. As part of this collaborative and client-centered team, you will work alongside healthcare providers, housing specialists, and community partners to reduce barriers, connect individuals with meaningful resources, and support access to safe, appropriate, and permanent housing.
In this role, you will manage a caseload and work directly with clients to establish and achieve personal goals—particularly those related to maintaining safe, stable housing as a foundation for long-term well-being. You will act as a strong advocate for clients, coordinating care and collaborating with a wide range of community partners, including emergency shelters, healthcare providers, income support services, and the justice system. Your work will be grounded in a trauma-informed, culturally safe, and compassionate approach that reflects the HART Hub’s commitment to low-barrier, integrated care.
Key Responsibilities:
Provide direct, client-centered support focused on housing stability and long-term recovery
Collaborate with interdisciplinary partners to develop and implement integrated care plans
Advocate for clients across systems, including health, housing, income, and justice
Ensure the safety and well-being of clients, colleagues, and the broader community
Participate in program development and continuous improvement initiatives
Complete administrative tasks and documentation in collaboration with team members and community stakeholders
The role is community-based and you will be expected to regularly travel within the London community. The schedule for the HART Hub would be working mainly Monday – Friday, 9:30m – 6:00pm, with one weekend from 10am – 6pm in a 4-week rotation.
Required Qualifications:
Post secondary education from an accredited institution
1-3 years’ experience working with a diverse population, including individuals impacted by mental illness, justice system involvement, addictions and homelessness
Standard First Aid / CPR with AED - Level C
Valid G2 or G Class Driver’s License and access to reliable vehicle
Candidates who are proficient in both official languages of Canada will be considered an asset
