Our vision is to be the premier healthcare provider in the region, delivering exceptional patient experiences through innovative, integrated care.<br><br>Essential Functions:<p><u>Quality Improvement&#160;</u>&#160;</p><ul><li>Partner with clinic leadership teams to develop, implement, and monitor site-specific quality and population health improvement plans.</li><li>Drive and support QI projects using data-driven methodologies such as Plan-Do-Study-Act (PDSA) cycles, Lean, and Six Sigma.</li><li>Monitor QI projects and report on progress towards meeting UDS, HEDIS, and PCMH measures.</li><li>Partner with Quality Improvement department staff to help drive quality improvement initiatives related to patient outcomes and patient satisfaction.</li><li>Partner with clinic leadership to drive clinic level quality improvement initiatives.</li><li>Facilitate clinical quality meetings, collaborate with providers and staff, and present findings with recommendations for improvement.</li><li>Assist in generating QI reports, dashboards, and educational tools for clinic providers/staff, leadership, and other stakeholders.</li><li>Assist in the maintenance of NCQA Patient-Centered Medical Home (PCMH) recognition.</li><li>Educate TrueCare staff on quality improvement principles and tools for implementing and improving patient outcomes.</li><li>Support risk management and case management activities when applicable.</li></ul><p><u>Population Health&#160;&#160;</u></p><ul><li>Improve care delivery processes by analyzing trends and developing QI interventions to identify and address health disparities, care gaps, and trends in chronic disease management, preventive care, and social determinants of health.</li><li>Work with Quality Improvement and Population Health leadership to support interventions aimed at improving outcomes in key populations (e.g., patients with diabetes, hypertension, behavioral health needs).</li><li>Collaborate with care management and clinical teams to design and evaluate outreach and care coordination programs.</li><li>Support initiatives to increase patient engagement in preventive services (e.g., screenings, immunizations, wellness visits).</li><li>Contribute to the development of risk stratification models and targeted outreach strategies</li></ul><strong>Qualifications:</strong><ul><li>Bachelor&#8217;s degree from an accredited institution in nursing, a healthcare related field, or an equivalent combination of education and professional experience in a related field.</li><li>Minimum of three (3) years of professional experience in a healthcare setting.</li><li>Minimum of one (1) year of professional experience working with quality improvement.</li><li>Experience working with quality improvement data and quality improvement methodologies (Plan Do-Study-Act, root cause analysis, etc.).</li><li>Computer proficiency with Microsoft Office suite products, including Outlook, Word, Excel, and PowerPoint</li><li>Will be required to travel between TrueCare locations therefore a valid California Driver's License, proof of valid automobile insurance and a clean driving record is required.&#160;</li></ul><strong>Preferred Qualifications:</strong><ul><li>Professional experience in community health.</li><li>Lean Six Sigma Certification.</li><li>Experience working in a Federally Qualified Health Center (FQHC).</li><li>Project Management Professional (PMP) certification</li></ul>The pay range for this role is $80,850 to a maximum of $ 121,274, during the lifespan of this title.&#160;&#160;<span style=\"font-size:12pt;\"><span style=\"line-height:115%;\"><span style=\"font-family:Aptos, sans-serif;\"><span style=\"font-family:Arial, sans-serif;\">&#8203;</span></span></span></span><p>Pay transparency: If you are hired at TrueCare, your salary will be determined based on factors such as education, knowledge, skills, and experience.
