This position facilitates coordination between prescribers, patients, pharmacy services (including 340B), and financial assistance programs to ensure timely, affordable access to treatment.</p><p>The Patient Access Specialist functions as a liaison to promote medication adherence by providing patient education, resolving insurance or cost-related barriers, assisting with prior authorizations, and supporting prescription renewals and refills. In collaboration with the care team, the Patient Access Specialist also identifies and addresses social determinants of health that may impact access to care or adherence.</p><p>Additionally, the role supports therapeutic outcomes by monitoring medication access issues, guiding providers through formulary and coverage requirements, and connecting patients to manufacturer copay and patient assistance programs.
