    "description": "<h2>Department Overview</h2>\n<p>Family Medicine at Scappoose is a primary care Family Medicine clinic and Rural Health Center (RHC). The patient population includes pregnant people, babies, children, adolescents, adults, and geriatrics. The patients are generally moderately stable to moderately unstable, with complex medical conditions and SDOH barriers that require medical and nursing interventions. These include, but are not limited to, oral and injectable medications, foot care, telephone and in-clinic triage, care coordination, health education, complex wound care, pain management, and behavioral and/or crisis management. </p>\n<h2>Function/Duties of Position</h2>\n<p><strong>This position will cross-train to support additional primary care clinics within the Department of Family Medicine and the Division of Internal Medicine. Responsibilities include remote phone triage, MyChart message management, results review and follow-up, transitions of care, and other duties aligned with operational needs to support clinic workflows and the RN team.</strong></p>\n<p><strong> </strong></p>\n<p> </p>\n<p>Nurses perform patient visits, telephone triage and advice, and manage an in-basket workload in EPIC where pts. are reviewed for Care Management, Care Coordination, and follow up for hospital and ED discharges.  </p>\n<p> </p>\n<p>Visit types offered at Family Medicine at Scappoose for all providers are: In-person, virtual, and telephone. </p>\n<p> </p>\n<p>Provider Services Offered:</p>\n<ul>\n <li>Primary Care</li>\n <li>Behavioral health and counseling services</li>\n <li>Reproductive health services</li>\n <li>Transgender health care</li>\n <li>Podiatry and foot care clinics</li>\n <li>Prenatal care (including deliveries)</li>\n <li>Pediatric care</li>\n <li>Medication-assisted treatment (MAT)</li>\n <li>Sports medicine</li>\n <li>Minor procedures</li>\n <li>Health maintenance with preventive care</li>\n</ul>\n<p><strong> </strong></p>\n<p>Most nursing work occurs outside of scheduled visits and services include</p>\n<ul>\n <li>Nurse telephone triage, with some in-clinic Nurse triage</li>\n <li>OB intake and support visits</li>\n <li>Wound Care</li>\n <li>IV visits</li>\n <li>Patient education</li>\n</ul>\n<h2>Required Qualifications</h2>\n<ul>\n <li>1 year experience working in ambulatory, care management or telephone triage experience in the last three years</li>\n <li>BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date.</li>\n <li>ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date.</li>\n <li>Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire</li>\n <li>Current, unencumbered Oregon State Registered Nurse License</li>\n <li>BLS from AHA required. New hires will be enrolled and required to complete during orientation.</li>\n <li>Must be able to perform the essential functions of the position with or without accommodation</li>\n</ul>\n<p> </p>\n<p> </p>\n<h2>Preferred Qualifications</h2>\n<ul>\n <li>National nursing professional certification in Ambulatory Nursing or other relevant specialty-population</li>\n <li>Experience in primary care, FHQC, or community clinic</li>\n</ul>\n<h2>Additional Details</h2>\n<p>Length of Orientation – New Graduate Nurse</p>\n<ul>\n <li>External candidates: OHSU & Nursing New Employee Orientation (NEO) for about a week.</li>\n <li>Either Transition to Practice (TTP) Program Residency Learning Pathway or Orientation experience for about 10-26 weeks depending on the care area. An employment service agreement may apply.</li>\n</ul>\n<p> </p>\n<p>Length of Orientation – Experienced Nurse</p>\n<ul>\n <li>External candidates: OHSU & Nursing New Employee Orientation (NEO) for about a week.</li>\n <li>Either Transition to Practice (TTP) Program Specialty Fellowship/Fellowship Learning Pathway or Orientation experience for 3 days-26 weeks depending on the care area.
