Evergreen Family Medicine serves outpatient needs, including Urgent Care, Family Practice, Women’s Health, Occupational Health, and school-based telehealth.</span></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif; font-size: 12pt\"><br>Evergreen Family Medicine is a Drug Free Workplace. All candidates that are offered employment will be required to pass a pre-employment drug screen and background check.</span></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif; font-size: 12pt\"><br><span style=\"font-weight: bold\">Job Summary: </span>Provides brief and ongoing assessment, intervention, and onsite, community or telehealth consultation to the primary care team regarding psychiatric/psychosocial concerns and health behavior change. While coordination of care is part of the job duties, the majority of the Behavioral Health Consultant’s activities will be focused on providing clinical interventions according to empirically supported treatment modalities.</span></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif; font-size: 12pt\"><br><span style=\"font-weight: bold\">Responsibilities and Duties:</span></span></p>\n<p>• Provides on-site and telehealth behavioral health services in primary care clinic.<br>• Maintains a schedule and presence that allows for immediate access by primary care providers. Promptly responds to provider calls and inquiries.<br>• Provides initial assessment and/or behavioral intervention with patients referred by primary care provider(s)<br>• Focuses assessment and/or intervention on referral issue/question/concern.<br>• Adheres to the principles of brief cognitive behavioral therapy and uses evidence-based or best practices when conducting behavioral interventions.<br>• Directs interventions toward helping patients develop self-management skills.<br>• Provides immediate feedback, verbal and/or written to primary care providers.<br>• Conducts brief, episodic follow-up interventions on a short-term basis as needed.<br>• Collects and utilizes data from screening tools for patient care.<br>• Provides crisis intervention and evaluation services as needed.<br>• Maintains current resource list and facilitates referrals as appropriate.<br>• Studies current literature on behavioral interventions with medical clinic patients and learns new techniques that are approved by Evergreen Family Medicine for use in this project.<br>• Assesses care needs of clinic population and works with providers, Clinic Administrator and Project Administrator to develop relevant program services (including involvement with support groups)</p>\n<p><span style=\"font-family: arial, helvetica, sans-serif; font-size: 12pt\"><br></span></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif; font-size: 12pt\"><span style=\"font-weight: bold\">Qualifications and Skills:</span><br>• Master’s degree in psychology, social work, or counseling<br>• Qualified Mental Health Assistant (QMHA) certified.<br>• Registration with the State of Oregon as an LPC Associate or Clinical Social Work Associate (CSWA).</span></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif; font-size: 12pt\"><br><span style=\"font-weight: bold\">Working Conditions and Physical requirements:</span><br>• Must attend provider meetings and other collaborative meetings, such as QAPI and PCPCH.<br>• Must be able to work flexible hours.<br>• Physical requirements for this position include hearing, seeing, speaking, feeling, talking, stooping, kneeling, crouching, reaching, grasping, and repetitive motions.<br>• This position requires sitting for long periods of time and walking and/or standing is only required occasionally.<br>• This position requires exerting up to 20 pounds of force and/or 10 pounds frequently in order to    move objects.<br>• May experience potentially assaultive clients and family members.</span></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif; font-size: 12pt\"><br>Our culture and values are every employee’s responsibility: The needs of our patient come first S.P.I.R.I.T<br>• Stewardship<br>• Patient &amp; Population Focused Health Care<br>• Integrity<br>• Respect<br>• Innovation<br>• Teamwork</span></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif; font-size: 12pt\"><br><span style=\"font-weight: bold\">Benefits:</span><br>• Health, Dental, Vision benefits<br>• Life Insurance<br>• 401k with a company match up to 6%<br>• Paid Time Off</span></p>",
