Apply now and be a part of something extraordinary at Johns Hopkins Intrastaff!</p>\n<p> </p>\n<p> </p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Conduct comprehensive biopsychosocial assessments</li>\n <li>Identify patient needs and develop individualized care plans and interventions</li>\n <li>Provide crisis counseling, suicide assessment, and short-term counseling</li>\n <li>Support care coordination and address social determinants of health</li>\n <li>Assist with complex discharge planning and mitigate psychosocial barriers</li>\n <li>Facilitate patient and family education and engagement</li>\n <li>Lead and participate in family meetings and advance care planning discussions</li>\n <li>Provide support around palliative care, end-of-life planning, grief, and loss</li>\n <li>Collaborate with interdisciplinary teams to optimize patient care</li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>MSW Social Work from an accredited school of social work.</li>\n <li>Must be licensed by the Maryland Board of Social Work Examiners at the LMSW or LCSW-C level.</li>\n <li>Must have Epic Experience </li>\n <li>Minimum of 2 years of experience in providing clinical social work service in a Oncology, pallative care, or Hospice Hospital setting is required.</li>\n <li>Experience with hospital discharge planning is a requirement.</li>\n</ul>\n<p> </p>\n<p>Johns Hopkins Health System and its affiliates are an Equal Opportunity / Affirmative Action employers.
