          "text": "<p>Current  </p><p> </p><p>QUALIFICATIONS:</p><p>Registered Nurse with current license in the state of Wisconsin/Illinois; one year of experience as an RN in an acute care setting, preferably two years of experience.Current CPR. Reliable car with insurance.  Home Health Care experience is desired.</p><p> </p><p></p><p><b> </b></p><p><b>ESSENTIAL FUNCTIONS: </b>(Other duties as assigned)</p><ol><li>Performs assessments and develops individualized patient care plans and manages multi-disciplinary care as applicable.</li><li>Provides skilled nursing care in a patient’s home as ordered by the attending physician in conjunction with the plan of care.</li><li>Supervises LPN’s and Home Health Aide’s in a      patient’s home.</li><li>Complies with agency policies and procedures.  Promotes and maintains  an agency  environment that is in compliance with federal, state, and local regulations.</li><li>Submits clear and appropriate documentation in a  timely manner and documents patient education.</li><li>Communicates effectively to physicians, supervisors, and coworkers in a timely manner.</li><li>Participates in case conferences to promote coordination of care.</li><li>Promotes continuity of care with appropriate admissions, transfers and discharges.</li><li>The RN will attend in-service training and attend mandatory agency meetings.</li><li>Reports to appointments as scheduled.</li><li>Maintains a professional appearance</li><li>Perform all aspects of patient care.</li></ol><p></p><p></p>",
