Additionally, we support health care providers who serve minority populations, learn how to improve their practice efficiencies, and optimize revenues to keep their doors open and expand access.</span></p>\n<p><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\"> </span></p>\n<p><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt; font-weight: bold\">Position Summary</span></p>\n<p><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\">The Outreach Engagement Specialist is responsible for serving as the initial and main point of Field contact between the Client and current Enrollees while ensuring the appropriate utilization of services that assist in achieving desired health outcomes.</span></p>\n<p><br></p>\n<p><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\"><span style=\"font-weight: bold\">Scope: Community-based </span><span style=\"font-weight: bold\"><span style=\"color: #2d2d2d\">Face-to-Face Outreach in Washington, DC</span></span></span></p>\n<p><br></p>\n<p><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt; font-weight: bold\">Responsibilities:</span></p>\n<ul>\n<li><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\"> Complete Initial screenings for any Enrollees who are within the ninety (90) day window of enrollment.</span></li>\n</ul>\n<ul>\n<li><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\">Problem solve and address barriers, recommending and engaging resources as appropriate to address identified risk factors through best practice and evidence-based interventions.</span></li>\n<li><span style=\"font-size: 12pt\"><span style=\"font-family: 'book antiqua', palatino\">Record details of attempts, interactions, services rendered to the Enrollee, and future recommendations.</span> </span></li>\n<li><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\">Support Enrollee access to care through home visits, processing of reports, and distribution of collateral materials. </span></li>\n<li><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\">Perform new member orientations. </span></li>\n<li><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\">Provide superior, professional, and courteous service to customers. </span></li>\n<li><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\">Participate in required trainings and monthly meetings.</span></li>\n<li><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\">Meet and exceed established performance metric standards.</span></li>\n</ul>\n<p><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt; font-weight: bold\"><br>Knowledge, Skills, and Abilities</span></p>\n<ul>\n<li><span style=\"font-size: 12pt\"><span style=\"font-family: 'book antiqua', palatino\">Ability to understand the basic principles of managed care and the concepts of publicly financed health insurance, such as Medicaid and SCHIP programs, is preferred.</span><br><br></span></li>\n</ul>\n<p><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt; font-weight: bold\">Qualifications:</span></p>\n<ul>\n<li><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\"> <span style=\"color: #2d2d2d\">High School Diploma or GED required.</span></span></li>\n<li><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\"> <span style=\"color: #2d2d2d\">Community Health Worker designation preferred.</span></span></li>\n<li><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\"> 1 + years of experience in customer service or any combination of education and experience that would provide an equivalent background. </span></li>\n<li><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\"> Must have self-motivation and initiative with the ability to work independently.</span></li>\n<li><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\">Incoming new hires will need to provide verification of their vaccination status within 60 days of hire.</span></li>\n<li><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\">Must possess and maintain a valid driver’s license with a clean driving record and a personal vehicle for transportation to/from the job site.<br> </span></li>\n</ul>\n<p><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\"> </span></p>\n<p><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt; font-weight: bold\">EEO Statement</span></p>\n<p><span style=\"font-family: 'book antiqua', palatino; font-size: 12pt\">Wellcentric DC is an Equal Opportunity Employer (EOE).
