Suburban Home Health is currently seeking a Physical Therapist to serve our adult and geriatric patients in Fayette county and the surrounding areas.&nbsp;</span><span id=\"isPasted\" style=\"background-color: transparent; font-family: tahoma, sans-serif; font-size: 16px;\">Suburban Home Health is a joint venture between several Suburban Health Organization hospital systems. &nbsp;We pride ourselves on having the best staff who provide the best possible care to our patients.</span><span style=\"background-color: transparent; font-family: tahoma, sans-serif; font-size: 16px;\">&nbsp;&nbsp;</span><div class=\"fr-view\"><div><div class=\"fr-view\"><p><br></p><p><span style=\"font-family: tahoma, sans-serif; font-size: 16px;\"><strong>Job Description</strong></span></p><p><span style=\"font-family: tahoma, sans-serif; font-size: 16px;\">The Physical Therapist assesses and treats patients in the home in order to assists them in the development and execution of activities of daily living.</span></p><ul><li style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Evaluates patients and develops a rehabilitation plan within the parameters prescribed by the referring physician to improve or restore physical functioning, alleviate pain and prevent physical dysfunction.</li><li style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Documents patient progress and communicates with other clinicians such as occupational therapy, speech therapy and skilled nursing on the status of patient&rsquo;s treatment plans.&nbsp;</li><li style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Administers active and passive exercises to help relieve pain, increase patient strength or decrease or prevent deformity.</li><li style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Directs physical therapist assistants and home health aides on the course of treatment for designated patients</li><li style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Educates and instructs patients, families and caregivers regarding treatment plans and procedures.&nbsp;</li><li style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Plans discharge when patient goals have been attained and communicates proper follow-up care to patient and caregivers.</li><li style=\"font-family: tahoma, sans-serif; font-size: 16px;\">&nbsp;Adheres to established organizational and departmental policies, standard operating procedures, quality assessment, and safety guidelines. &nbsp;</li><li style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Displays and maintains behavior which promotes a positive experience for patients, caregivers, referring physicians and co-workers.</li></ul><p><span style=\"font-family: tahoma, sans-serif; font-size: 16px;\"><strong>Basic Qualifications</strong></span></p><ul><li style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Must be a graduate of an accredited school of physical therapy and holder of a current permanent or temporary license from the Indiana Medical Licensing Board</li><li style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Ability to demonstrate knowledge and technical skill necessary for providing competent, comprehensive patient care.&nbsp;</li><li style=\"font-family: tahoma, sans-serif; font-size: 16px;\">CPR certification required.&nbsp;</li><li style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Must possess an Indiana Driver&#39;s license and automobile liability coverage as required by the State of Indiana.</li><li style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Ability to travel to all regions serviced by the agency in various weather conditions</li><li style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Must have the ability to perform essential job functions with or without reasonable accommodation</li></ul><p><br></p><p><span style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Suburban Home Health is an Equal Opportunity Employer and prohibits discrimination and harassment of any kind.
