CASE MANAGER – MOBILE CRISIS RESPONSE TEAM (MCRT) (CONTRACT) (33-26)
Job Status: Contract until March 2027
Hours of Work: 35 hours per week
Base office: Bracebridge
Competition: 33-26
Purpose of the Position: The Case Manager is responsible for carrying out case management, psychosocial rehabilitation, crisis intervention and treatment for individuals living with mental illness. This team member has the clinical skills and experience necessary to assess, plan, develop, co-ordinate, and provide treatment by way of rehabilitative and supportive services in a multi-disciplinary environment. The Case Manager develops supports and opportunities for clients to increase or develop new skills needed to support success and satisfaction in their daily living, learning, working and social environments with the least amount of professional intervention required.
Responsibilities extend beyond traditional office hours, encompassing telephone-based support, mobile outreach response, and in-person interactions. The clinician is also required to manage referrals efficiently and provide immediate and accessible information to consumers accessing our program. This role involves active participation in a 24/7 on-call service, ensuring continuous support for those in need. The Case Manager will work in partnership with the Mobile Crisis Response Team (MCRT) and with OPP.
Education and Qualifications:
Bachelor’s degree in a human service/health science discipline relevant to the position. Equivalency may be considered for a combination of educational experiences. Examples of equivalency include;
Mental Health related diploma with relevant work experience PLUS registration with a regulated health professional college
One year of experience working with persons who have mental illness and/or developmental disability and/or substance use issues preferred, plus experience in providing one or more of the following;
supportive counselling
case management
crisis intervention
Indigenous culture education and awareness is an asset.
Demonstrated knowledge of clinical case management principles, techniques and their application to specific casework, in particular regarding crisis intervention, is preferred.
Demonstrated knowledge of the operation of agency services and community programs/resources.
Knowledge of mental illness and issues related to severe mental illnesses, including their impact upon social, living, learning and earning aspects.
Ability to provide effective clinical assessment and service during day-to-day interactions with consumers, staff, and volunteers through consultation, modelling, and demonstration of effective service standards and agency philosophy. .
Ability to assist in decision making regarding consumer need and triage in a clinical team setting.
Ability to work autonomously and with a multidisciplinary team in a rural setting.
A willingness to serve consumers within the communities of the Districts of Muskoka and Parry Sound.
Experience with Microsoft Word, Outlook Calendaring and consumer information systems.
A valid Ontario G or G2 driver’s license and a safe reliable means of transportation.
A willingness to work shifts (including evenings, weekends and statutory holidays).
Bilingual in French is an asset.
Salary scale: $29.85 to $39.09 per hour commensurate with qualifications and experience
