    "requisitionDescription": "<div id=\"isPasted\"><p>Want to see it all? Work on Med-Surg and the ICU, see a couple patients in the ED, and help a new mom recover from a c-section, all in one day? Then this position is for you! Enjoy the flexibility of working 3 days a week and having the weekdays to yourself! &nbsp;The shift is for inpatient rehab including Med/Surg and ICU as well as the Emergency Department and Obstetrics for post-partum care, Saturday/Sunday/Monday with occasional holiday coverage, weekend and holiday differential available. We are one of the largest critical access hospitals in the region based on services provided and are a Level III Adult Trauma Center. We have a large physician group consisting of primary care providers and specialists including orthopedics, women&#39;s health, interventional pain, spine surgery, and neurology. &nbsp;We are looking for a therapist who can work independently and shift focus when needed to a different area of the hospital. One who puts the needs of their patients above all else, demonstrates accountability and integrity, and takes initiative in continuously striving to be the best provider they can be for their patients.&nbsp;</p><p><br></p><p data-pasted=\"true\"><strong>SUMMARY AND ESSENTIAL FUNCTIONS:</strong></p><p>Licensed Physical Therapists are responsible for the delivery of skilled patient care in a safe and timely manner to all of those referred for services. &nbsp;The Physical Therapist understands and adheres to all hospital and departmental policies and procedures, as well as complying with the New Hampshire Physical Therapy Practice Act (Section 328-A:2).</p><p>&nbsp;</p><ol start=\"1\" type=\"1\"><li>Clinical Evaluation and Assessment: Conduct comprehensive evaluations and assessments of patients&#39; musculoskeletal and neuromuscular conditions, pinpointing functional limitations and developing individualized treatment plans in accordance with New Hampshire&#39;s physical therapy regulations.</li></ol><ol start=\"2\" type=\"1\"><li>Treatment Planning and Implementation: Formulate and execute evidence-based physical therapy treatment plans tailored to each patient&#39;s specific needs, promoting optimal rehabilitation and functional improvement while adhering to state practice guidelines.</li></ol><ol start=\"3\" type=\"1\"><li>Rehabilitation and Functional Restoration: Focus on restoring patients&#39; physical function, mobility, strength, and coordination through therapeutic interventions, contributing to their overall well-being and independence.</li></ol><ol start=\"4\" type=\"1\"><li>Patient Education: Educate patients and their families on prescribed exercises, self-care strategies, and injury prevention techniques, ensuring that patients are well-informed and capable of managing their conditions effectively.</li></ol><ol start=\"5\" type=\"1\"><li>Collaboration and Communication: Collaborate closely with an interdisciplinary healthcare team, including physicians, nurses, occupational therapists, and other healthcare professionals, to ensure coordinated patient care and successful treatment outcomes, in alignment with the collaborative practices emphasized in New Hampshire&#39;s physical therapy regulations.</li></ol><ol start=\"6\" type=\"1\"><li>Inpatient Care: Provide physical therapy services in inpatient settings as needed, such as acute care, emergency department, same-day surgery, obstetrics unit, and/or the intensive care unit, actively participating in patient rounds and discharge planning to facilitate smooth transitions.</li></ol><ol start=\"7\" type=\"1\"><li>Documentation and Record Keeping: Maintain thorough and accurate patient records, including evaluation findings, treatment plans, progress notes, and discharge summaries, ensuring that documentation is compliant with the state&#39;s physical therapy regulations and legal requirements.</li></ol><ol start=\"8\" type=\"1\"><li>Continuing Education and Professional Growth: Engage in ongoing professional development to stay current with the latest advancements in the field of physical therapy, attending workshops, seminars, and relevant courses to enhance your clinical skills and knowledge.</li></ol><ol start=\"9\" type=\"1\"><li>Compliance and Regulatory Adherence: Strictly adhere to all relevant laws, regulations, and ethical standards governing the practice of physical therapy in New Hampshire, upholding the highest levels of professional and ethical integrity.</li></ol><table border=\"0\" cellspacing=\"0\" cellpadding=\"0\"><tbody><tr><td valign=\"top\" style=\"width: 100%;\"><p>&nbsp;</p><p>&nbsp;</p></td></tr></tbody></table><p><strong>REPORTS TO:&nbsp;&nbsp;</strong>Director of Rehab Services</p><p>&nbsp;</p><p><strong>SUPERVISES:&nbsp;&nbsp;</strong>A licensed Physical Therapist supervises the care given by a Licensed Physical Therapy Assistant and a Physical Therapy/Physical Therapy Assistant Student. &nbsp;A Physical Therapist also directs and supervises all tasks designated to a Rehabilitation Aide and other support personnel.</p><p>&nbsp;</p><p><strong>INTERNAL AND EXTERNAL CONTACTS:&nbsp;</strong>Patients and families, Hospital Personnel, Physicians and office Staff, Financial and Legal Representatives, Community Members, Contracting Agencies, Students and Academic Coordinators, Home Health, and Co-workers<strong>.&nbsp;
