Organization Summary:
Community Care Cooperative (C3) is a 501(c)(3) non-profit, Accountable Care Organization (ACO) governed by Federally Quality Health Centers (FQHCs). Our mission is to leverage the collective strengths of FQHCs to improve the health and wellness of the people we serve. We are a fast- growing organization founded in 2016 with 9 health centers and now serving hundreds of thousands of beneficiaries who receive primary care at health centers and independent practices across Massachusetts. We are an innovative organization developing new partnerships and programs to improve the health of members and communities, and to strengthen our health center partners.
Job Summary:
The Senior Program Manager, Primary Care Innovation plays a lead role in supporting health centers in developing the capabilities in primary care to be successful in a value-based environment. The Senior Program Manager will work with other members of the Primary Care Innovation team to foster improvement opportunities and promote innovation efforts at health centers to drive success in value- based care and improve primary care delivery at health centers. The Senior Program Manager will play a lead role in monitoring industry trends, assessing digital health solutions and other tech-enabled model of care opportunities, leveraging data to identify opportunities to impact quality and total cost of care, piloting and evaluating the impact of new initiatives, and documenting and disseminating learnings. The Senior Program Manager will leverage communication skills, critical thinking capabilities, and on-the- ground experience in primary care innovation to advance C3’s strategic priorities.
The Senior Program Manager will report to the Director, Primary Care Innovation. The Senior Program Manager will collaborate effectively across teams and stakeholders while also demonstrating strong ownership and the ability to work independently in a fast-paced environment.
Responsibilities:
Monitoring, Assessing, and Communicating Industry Trends:
Monitor and convey trends relevant to the fields of value-based care, AI and digital health, and primary care transformation Serve as the organizational representative for relevant collaborative groups and partnerships Serve as the point person for coordinating the evaluation of emerging digital health and AI solutions and other technology-enabled model of care opportunities relevant to the field of primary care; in partnership with other relevant departments and leaders, make recommendations to C3 leadership about opportunities to prioritize and pursue Developing & Testing New Care Delivery Models
Use data-driven approach to identify and assess opportunities that help health centers succeed in value-based care and improve primary care delivery, including initiatives that reduce provider burnout and strengthen operational efficiency Partner with health center teams and C3 colleagues to design, pilot, and implement innovative care delivery models within health centers, such as team-based care, redesigned workflows, or tech-enabled care models Collaborate with health center teams, C3 analytics, and other stakeholders to evaluate impact of new policies and innovations on quality, cost, and care team satisfaction Communications & Strategic Partnerships
Document and disseminate key learnings and develop tools and resources for health centers (blog posts, case studies, tip sheets, online toolkits, etc.) Use communication strategies and expertise to advance strategic priorities at C3 Collaborate with external stakeholders, including funders and other strategic partners, to advance priorities Primary Care Innovation Operations
Support and/or lead the planning, management, and facilitation of forums and workgroups to facilitate peer sharing and learning across health centers (which may include the Improvement and Innovation Forum, Primary Care Innovation Advisory Council, CMO Forum, AI Subcommittee, and others) Partner with other members of the Primary Care Innovation team in ongoing program operations (e.g., monitoring and disseminating data and reports to health centers, preparing presentations, grant reporting) Provide ongoing technical assistance in the fields of team-based care, primary care transformation, and AI and digital health to health center teams Manage relationships with vendors and other external partners Manage relevant grants and ensure grant deliverables are met on time and in scope Remain up to date on the general fields of primary care innovation, digital health and AI, and value-based care Required Skills:
A strong commitment to the organization’s mission Strong interpersonal and relationship building skills Ability to communicate dexterously across a diverse range of stakeholders, from health centers leadership to primary care and support staff responsible for driving the success of strategic programs in health centers Ability to work collaboratively, across the organization, and as part of a team Project management expertise Strong oral and written communication skills Experience with quality and performance improvement approaches/techniques Self-starter; exercises high degree of initiative, judgement, discretion and decision making to achieve objectives Comfortable multi-tasking Performs with great integrity and produces accurate work with close attention to detail, especially in the completion of final deliverables to internal and external stakeholders Comfortable with ambiguity: seeks new data and can make recommendations based on best available data Desired Other Skills:
Experience working on a primary care team or leading primary care teams through teams through change is preferable Familiarity with emerging AI and digital health technologies and interest in evaluating their potential applications in primary care Familiarity with the MassHealth ACO program and/or other value-based care models Familiarity with Federally Qualified Health Centers Experience with anti-racism activities, and/or lived experience with racism is highly preferred Qualifications:
Bachelor’s Degree required Previous experience in health center or primary care operations preferred
