The RPN role includes medication administration, delivering patient care to individuals who are admitted due to serious psychiatric conditions or symptoms. Your responsibilities include: Performing behavioral observations, mental status assessments, and physical assessments on all assigned patients Administering medications and monitoring patients' responses to pharmaceutical care interventions Maintaining accurate and timely documentation of patient care, observations, and interventions in accordance with organizational standards Providing ongoing communication and collaboration with charge nurses, physicians, and other interdisciplinary team members regarding patient progress and care needs Implementing therapeutic nursing interventions and building therapeutic relationships with patients to support recovery and engagement in treatment Monitoring and promoting patient safety, including suicide and harm prevention precautions, and responding promptly to behavioral changes or crisis situations Supporting patients in medication compliance and providing education regarding psychiatric medications and treatment plans Participating in discharge planning activities and patient advocacy to ensure continuity of care and successful community reintegration Collaborating with the interdisciplinary team to contribute to individualized treatment planning and goal-setting for assigned patients Halton Healthcare endeavors to hire individuals who are competent in their chosen profession and who exemplify our mission, vision and values of accountability, respect and compassion which guide our individual behaviours and attitudes.
