About the Role
Help people rediscover their ability to do the things that matter most. As a Home Health Occupational Therapist, you'll work one-on-one with patients in their homes - teaching new skills, retraining lost abilities, and creating innovative solutions that enable them to regain independence in their daily activities. This role is perfect for a licensed OT who loves the creativity and problem-solving inherent in occupational therapy and wants the time to truly make a difference. You'll assess each patient's unique home environment, adapt treatments to their specific needs, and watch as they regain the ability to dress themselves, cook meals, and participate in the activities that give their lives meaning.
What You'll Do
Restore Meaningful Function
Teach new skills or retrain patients in once-familiar daily activities lost due to illness or injury
Provide therapeutic activities designed to restore function and self-care abilities
Demonstrate and teach alternate techniques for completing activities of daily living
Focus on improving fine motor skills, coordination, and cognitive abilities for functional tasks
Work toward patient and caregiver independence as the ultimate goal
Assess and Create Individualized Plans
Conduct comprehensive initial and ongoing assessments of functional level using appropriate tests
Develop and revise treatment plans in consultation with physicians and care team members
Set realistic, measurable goals that address what matters most to each patient
Evaluate home environments and identify modifications to support independence
Assess ongoing care and equipment needs throughout the episode
Provide Adaptive Solutions
Order and/or fabricate orthotic, adaptive, and assistive devices tailored to patient needs
Train patients and families in the use of adaptive equipment and assistive technology
Recommend home modifications and safety equipment to support independence
Teach proper transfer techniques and positioning strategies
Design creative solutions to overcome environmental barriers in the home
Educate and Empower
Instruct patients, families, and caregivers in exercise programs to increase strength and control
Teach energy conservation and work simplification techniques
Provide education on disease prevention and injury management
Counsel families on home rehabilitation procedures and ongoing care needs
Supervise and instruct occupational therapy assistants in patient-specific care plans
Collaborate with the Care Team
Coordinate with physicians, nurses, PTs, MSWs, and other healthcare professionals
Attend and initiate patient care conferences for complex cases
Make referrals to other disciplines when indicated by patient needs
Communicate patient progress, reactions to treatment, and condition changes
Participate in quality improvement initiatives and QAPI activities
Maintain Clinical Excellence
Complete accurate, timely documentation according to agency policies
Monitor and report changes in patient condition to physicians and care managers
Stay current with occupational therapy trends and evidence-based practices
Participate in discharge planning to ensure smooth transitions
Evaluate outcomes of care and adjust treatment approaches accordingly
What We're Looking For
Required Qualifications
Graduate of an accredited occupational therapy program (ACOTE accredited)
Current state license as an Occupational Therapist
Minimum 1 year of experience as a licensed Occupational Therapist
Reliable transportation with valid automobile insurance
Willingness to travel daily for home visits within service area
Preferred Qualifications
Home health experience
Experience with adaptive equipment and home modifications
Knowledge of Medicare home health documentation
Essential Skills
Strong clinical assessment and treatment planning abilities
Knowledge of medical terminology and home health regulations
Excellent problem-solving and creative thinking skills
Strong verbal and written communication abilities
Ability to work independently with minimal supervision
Organizational skills and attention to detail
Comfort working in varied home environments
Interpersonal skills for professional interaction with patients, families, and teams
What Makes You Successful
You're a creative problem-solver who sees possibilities where others see limitations. When a patient can't grip a fork, you don't just prescribe an adaptive utensil - you assess their hand function, consider their specific needs and preferences, and develop a comprehensive approach that might include strengthening exercises, modified techniques, and the right assistive device.
You're passionate about meaningful occupation - understanding that being able to button a shirt or cook a simple meal isn't just about physical function, it's about dignity, independence, and quality of life. You find deep satisfaction in the small victories that mean everything to your patients.
You excel at teaching and can break down complex tasks into manageable steps. You're patient and encouraging, celebrating progress while continuing to challenge patients toward greater independence. You understand that your success is measured not by how many visits you make, but by how well patients can function after you're gone.
You're comfortable with the autonomy of home health practice - managing your schedule, making clinical decisions independently, and adapting your approach to each unique home environment. You're resourceful, able to create effective treatments with whatever is available in the patient's home.
Why Join Us?
Home health occupational therapy offers unparalleled opportunities for meaningful, creative practice. You'll have the time to truly understand each patient's goals, the freedom to design innovative treatment approaches, and the privilege of seeing remarkable functional gains that transform lives.
We provide comprehensive orientation, ongoing clinical support from experienced managers, and opportunities for professional development. You'll work with a collaborative team that values your expertise in helping patients regain independence in the activities that matter most to them.
Professional Growth:
Continuing education opportunities
Clinical mentorship and consultation
Exposure to diverse patient populations and conditions
Training on adaptive equipment and home modifications
Participation in quality improvement initiatives
Work Environment and Schedule
This position requires daily travel to patient homes and residential facilities within your service area, with occasional office meetings. You'll work independently during home visits while staying connected to your care team through regular communication and care conferences.
Schedule flexibility is important as you'll adapt to patient needs, new admissions, and changing conditions. Most visits occur during standard business hours, though some flexibility for patient availability may be needed. Physical demands include standing, bending, demonstrating activities, and occasionally assisting with patient mobility.
Ready to Restore Independence?
If you're a licensed occupational therapist who wants the creativity and autonomy of home health practice and the satisfaction of helping patients reclaim their ability to perform meaningful daily activities, we'd love to hear from you.
