The Wound Care Nurse is responsible for assisting in the management and treatment of patients with chronic, non-healing wounds, ulcers and other skin conditions</em>.</span><p style=\"margin: 0in 0in 8pt; font-size: 11pt; font-family: Calibri, sans-serif; line-height: 1;\"><span style=\"font-size: 14px; font-family: tahoma, sans-serif;\"><br></span></p><p style=\"margin: 0in 0in 8pt; font-size: 11pt; font-family: Calibri, sans-serif; line-height: 1;\"><span style=\"font-size: 14px; font-family: tahoma, sans-serif;\"><strong>Essential Functions Include:</strong></span></p><ul style=\"list-style-type: disc;margin-left:undefined;\"><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Accurately performs and documents patient assessments and identifies patient needs.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Evaluates progress and outcomes.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Assesses, determines classification, stages, measures, and photographs wounds accurately and in accordance with policy.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Identifies and initiates appropriate nursing interventions.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Documents care plans and progress toward goals.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Provides patient and family teaching as it pertains to the treatment plan, procedures, and preventative measures throughout the course of treatment.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Provides direct care appropriate to condition and age of the patient.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Works cooperatively and actively participates with other members of the care team. Assists other team members to help balance the workload.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Participates in continuous quality and performance improvement. Maintains a positive attitude and remains patient focused.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Complies with regulatory and accreditation standards as well as Center policies and procedures.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Responsible for personal and professional development.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Maintains ethical practices and HIPAA requirements.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Performs timely and appropriate documentation in the Center&rsquo;s EMR.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Provides a safe environment for patients, visitors and staff and complies with appropriate and approved safety standards.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Collaborates as needed with other healthcare providers, referral sources, Center Manager, HBO Safety Director, and the Medical Director regarding clinic and patient needs.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Assists in inventory management by identifying supply needs in advance of depletion. Notifies clinic manager of clinic needs.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Performs case management as needed to ensure physician orders and referrals are carried out. Schedules outside referral appointments as necessary to ensure continuity of care.</li><li style=\"font-size: 14px; font-family: tahoma, sans-serif; line-height: 1;\">Ensures ordered test results and/or referrals are received prior to next clinic visit.
