Promotes recovery from mental illness.</p><p><br></p><p><strong>SUPERVISION</strong></p><p><br></p><p><strong>Supervision Received</strong></p><p>This position is supervised by the Practice Manager, but also works closely with the Community Counseling Solutions (CCS) Medical Director&nbsp;</p><p><strong>Supervision Exercised</strong></p><p>This position does not have any supervisory responsibilities.</p><p><br></p><p><strong>RESPONSIBILITIES</strong></p><ul><li>Completes psychiatric evaluations as required on new clients. Reevaluates and updates as needed.</li><li>Performs age-appropriate history and physical for complex acute, critical, and chronically ill perioperative patients.</li><li>Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs.</li><li>Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.</li><li>Implements interventions to support the patient to regain or maintain physiologic stability, including but not limited to serving in the first assisting role. Monitors the effectiveness of interventions.</li><li>Facilitates the patient’s transition within and between health care settings, e.g. admitting, transferring and discharging patients.</li><li>Reviews and evaluates clinical data pertaining to physical status of client.</li><li>Reviews and follows up on previous mental and physical treatment plans.</li><li>Leads prescribed individual, group therapy and/or education sessions as part of specific therapeutic procedures.</li><li>Reviews, approves and signs individual plans of service of clients.</li><li>Documents and updates client’s progress in progress notes.</li><li>Prescribes client medication. Monitors medication effectiveness and modifies as necessary.</li><li>Works with team members, including clinicians, nurses and social workers to discuss client progress and outlook. Coordinates with outside agencies as necessary. Act as a client advocate when appropriate.</li><li>Contacts client’s relatives to arrange family conferences if necessary.</li><li>Provides seclusion and restraint, de-escalation, and other facility related trainings to CCS employees.</li><li>Provides after hours services during regularly scheduled work week. This includes telephone consultation as needed with CCS employees and crisis intervention in the community and facility.</li><li>Responsible to work closely with supervisor to find back up when on vacation.</li><li>Completes all paperwork in a way that is professional, clinically sound and timely as prescribed by the OAR’s and other pertinent guidelines.</li><li>Other duties as assigned.</li></ul>",
