Description ESSENTIAL DUTIES AND RESPONSIBILITIES NYSSHP Coordinator will provide case management services to individuals and families with a history of homelessness, housing instability, or who have exited NYC shelters into permanent housing units across RiseBoros multi-family buildings in the neighborhoods of Bushwick and East New York. This position is often in the field as the NYSSHP Coordinator will travel between multiple buildings for home visits with clients in their caseload. The Coordinator will: Actively coordinate day to day social services for residents in NYSSHP units and ensure all service provision is representative of RiseBoros mission. Provide ongoing case management, assistance and advocacy, providing information and referrals to community services as needed. Assist clients with regards to benefit entitlements such as SSA, SSI, SNAP, Public Assistance, Medicaid, TANF, and others, conducting advocacy as needed. Educate residents on the available on-site services as well as those available in the community. Conduct regular outreach to clients via home visits and telephone calls. Prepare and maintain up to date records and clients files, including clients personal and eligibility information, records of services provided, and all relevant correspondence. Enter accurate and complete case notes in a timely fashion and contribute to periodic reports to funding agency. Develop service and goal plans with client to ensure housing stability. Establish individual and family goals to increase: money management skills, higher educational level, household income and education on tenant rights and responsibilities. Escort family members to entitlement appointments/certifications as needed and appropriate. Deliver services with sensitivity to the cultural and socioeconomic characteristics of all members of the family. Provide crisis intervention, de-escalation, and referrals to counseling services when necessary. Coordinate with community partners to organize educational and recreational workshops. Communicate with building management staff on tenant issues, including discussion of rent ledgers, rental subsidies, annual recertification, etc. Provide access to mediation services, as needed, in the event of rental arrears or violation of house rules. Educate clients on the available services in the community; make referrals when necessary and appropriate. Additional responsibilities may be added as position evolves. Perform other related duties as directed by management. COMPETENCIES Some experience in social service field required; 1-2 years of case management experience preferred Bi-lingual English/Spanish required Dynamic, able to multi-task and work independently Must be a team player and work effectively with culturally, economically, and educationally diverse populations. Must have the ability to problem solve and work with a high level of autonomy. Ability to communicate effectively and maintain good relations with clients and colleagues. Able to travel for home visits and appointments Commitment to RiseBoros mission, vision and values. Ability to understand the concepts of institutional and structural racism and bias and their impact on underserved and underrepresented communities Have a demonstrated commitment to supporting communities who have experienced systemic oppression and bias (e.g. people of color, LGBTQ+ people, immigrants, justice involved persons, etc.). The Coordinator will:\\r\\n\\r\\n\\r\\n\\tActively coordinate day to day social services for residents in NYSSHP units and ensure all service provision is representative of RiseBoros mission.\\r\\n\\tProvide ongoing case management, assistance and advocacy, providing information and referrals to community services as needed.\\r\\n\\tAssist clients with regards to benefit entitlements such as SSA, SSI, SNAP, Public Assistance, Medicaid, TANF, and others, conducting advocacy as needed.\\r\\n\\tEducate residents on the available on-site services as well as those available in the community.\\r\\n\\tConduct regular outreach to clients via home visits and telephone calls.  \\r\\n\\tPrepare and maintain up to date records and clients files, including clients personal and eligibility information, records of services provided, and all relevant correspondence.\\r\\n\\tEnter accurate and complete case notes in a timely fashion and contribute to periodic reports to funding agency.\\r\\n\\tDevelop service and goal plans with client to ensure housing stability.\\r\\n\\tEstablish individual and family goals to increase: money management skills, higher educational level, household income and education on tenant rights and responsibilities. \\r\\n\\tEscort family members to entitlement appointments/certifications as needed and appropriate.\\r\\n\\tDeliver services with sensitivity to the cultural and socioeconomic characteristics of all members of the family.\\r\\n\\tProvide crisis intervention, de-escalation, and referrals to counseling services when necessary.\\r\\n\\tCoordinate with community partners to organize educational and recreational workshops.\\r\\n\\tCommunicate with building management staff on tenant issues, including discussion of rent ledgers, rental subsidies, annual recertification, etc. \\r\\n\\tProvide access to mediation services, as needed, in the event of rental arrears or violation of house rules.\\r\\n\\tEducate clients on the available services in the community; make referrals when necessary and appropriate.\\r\\n\\tAdditional responsibilities may be added as position evolves.\\r\\n\\tPerform other related duties as directed by management.\\r\\n\\r\\n\\r\\nCOMPETENCIES\\r\\n\\r\\n\\r\\n\\tSome experience in social service field required; 1-2 years of case management experience preferred\\r\\n\\tBi-lingual English/Spanish required\\r\\n\\tDynamic, able to multi-task and work independently\\r\\n\\tMust be a team player and work effectively with culturally, economically, and educationally diverse populations.\\r\\n\\tMust have the ability to problem solve and work with a high level of autonomy.\\r\\n\\tAbility to communicate effectively and maintain good relations with clients and colleagues.\\r\\n\\tAble to travel for home visits and appointments\\r\\n\\tCommitment to RiseBoros mission, vision and values.\\r\\n\\tAbility to understand the concepts of institutional and structural racism and bias and their impact on underserved and underrepresented communities\\r\\n\\tHave a demonstrated commitment to supporting communities who have experienced systemic oppression and bias (e.g. people of color, LGBTQ+ people, immigrants, justice involved persons, etc.).\\r\\n\\r\\n\\r\\nEDUCATION EXPERIENCE\\r\\n\\r\\nAssociates degree required; Bachelors degree preferred\",\"employmentType\":\"FULL_TIME\",\"hiringOrganization\":{\"@type\":\"Organization\",\"name\":\"RISEBORO COMMUNITY PARTNERSHIP\",\"logo\":\"https://www.paycomonline.net/v4/ats/web.php/application/style/logo?clientkey=EADEA190247F3A600C15C7900B9DC6D1\"},\"jobLocation\":{\"@type\":\"Place\",\"address\":{\"streetAddress\":\"21 31 SCHAEFER STREET\",\"addressLocality\":\"BROOKLYN\",\"addressRegion\":\"NY\",\"postalCode\":11206,\"addressCountry\":\"USA\"}},\"qualifications\":\"It is the policy of RiseBoro to provide equal employment opportunity to all employees and applicants for employment.
