Case Manager – Recovery Support Services | Southeastern Connecticut | New London, CT
Southeastern Council on Alcoholism and Drug Dependence, Inc. We are seeking a compassionate and resourceful Case Manager to provide client placement, referral, and discharge planning services — connecting individuals in recovery with the resources, support, and pathways they need to build meaningful, lasting lives.

As a Case Manager, you will serve as a vital guide and advocate for clients navigating the recovery process — conducting intake interviews, developing intervention plans, coordinating community resources, and providing crisis support. You will work collaboratively across disciplines to ensure every client has access to a continuum of care tailored to their individual needs, goals, and strengths.
Responsibilities include:
Interview clients, complete intake information, review records, and confer with other professionals to evaluate client needs Use motivational interviewing to coach and empower clients toward positive outcomes Refer clients to additional support services as needed, including medical evaluation and treatment, social services, and employment services Apply professional judgment, in conjunction with assigned providers, to plan, develop, and implement effective intervention plans that support the most positive clinical outcomes Assist with treatment plan objectives through exploration of problems, examination of attitudes and feelings, consideration of alternative solutions, and decision making Coordinate case management needs to bring services, agencies, resources, and clients together to facilitate discharge planning and referrals Provide crisis intervention services to respond to clients' acute emotional and/or physical distress Provide education and information to individual clients and groups concerning alcohol and drug abuse, available treatment options, 12-step programs, and other community-based resources Meet regularly with counseling staff and other relevant stakeholders to report on client progress, and discuss and modify established treatment plans as needed Participate in the orientation of new employees and the transfer of knowledge essential to the performance of duties What You'll Need:
Associate's degree preferred; equivalent knowledge and experience in substance abuse and community support services will be considered Valid and unrestricted Driver's License, with the ability to drive an agency vehicle required Strong communication, organizational, and problem-solving skills Commitment to trauma-informed care, recovery-oriented practice, and SCADD's values of diversity, equity, and inclusion Ability to work both independently and collaboratively in a fast-paced environment Why SCADD?
