    "description": "<h2>Overview</h2>\n<p>Under the general direction of the Corporate Wellness Director and consistent with the Christian mission and the policies and procedures set forth by the <strong>YMCA of Catawba Valley </strong>at the <strong>Phifer Family </strong>branch, the <strong>Chronic Disease Coach</strong> will facilitate delivery of YMCA’s Evidence-based Health Initiatives (EBHI) Programs including, but not limited to Parkinson’s Program and LiveStrong at the Y, provide support and guidance to program participants, and maintain fidelity to the curriculum.</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Deliver class to program participants in effective, meaningful, and compelling ways. </li>\n <li>Encourage group participation and interaction through the use of open-ended questions and other facilitation skills. </li>\n <li>Cultivate participant commitment and support retention of participants. </li>\n <li>Create a safe, judgment-free, and encouraging environment that is friendly and non-competitive for participants.</li>\n <li>Foster relationships with and between participants. </li>\n <li>Make learning a shared responsibility for the group. </li>\n <li>Support and encourage participant goal-setting at each session.</li>\n <li>Travel to community locations for program delivery.</li>\n <li>Prepare for each class (e.g. LiveStrong: review session content, develop facilitation outline, review notes, conduct participant reminder call/send participant reminder email). </li>\n <li>Be accessible to participants before and after sessions to answer questions and provide follow-up. </li>\n <li>Follow-up with participants outside of class as needed. (LiveStrong only)</li>\n <li>Adhere to the YMCA’s HIPAA policies and procedures and ensure participants’ protected health information is secure. </li>\n <li>Participate in quality assurance opportunities to support fidelity to the curriculum and to meet performance standards. </li>\n <li>Arrive for class on time and dressed appropriately. </li>\n <li>Participate in all required LiveStrong or Parkinson’s Coach team discussions (meetings, emails, or conference calls) and professional development opportunities, offering ideas and tools for facilitation to other team members and remaining engaged at all times. </li>\n <li>Connect with Program Coordinator on a regular basis to update on program status and progress and as per any additional expectations outlined.</li>\n</ul>\n<p><strong>All YMCA staff are responsible for creating a safe, inclusive environment.
