This role combines safe transportation responsibilities with direct participant support to help individuals achieve personal goals, increase independence, and enjoy enriching life experiences. The ideal candidate is compassionate, dependable, flexible, and committed to creating a positive and inclusive environment for all participants. What You’ll Do Provide safe and reliable transportation for participants between their homes, the program center, and community activities Assist individuals who use wheelchairs or other mobility devices during transportation and activities Support participants during daily activities while encouraging independence and participation Assist with daily living needs including lunch support, toileting, mobility, and other personal care tasks while maintaining dignity and respect Foster a welcoming, supportive, and positive environment for participants Build strong, respectful relationships with participants, families, and team members Contribute observations and feedback to individual support plans Complete required documentation including daily notes and monthly reports accurately and on time Collaborate with team members and provide support wherever needed to ensure quality participant experiences
