Overview Brookhaven Hospice is currently hiring Per Diem Compassionate Licensed Practical Nurses (LPN) to provide patient care in and around the Hampden & Hampshires County Area! Our Licensed Practical Nurses (LPNs) are responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include, but are not limited to, following the plan of care, providing treatment, and working collaboratively with the members of the team to help meet positive patient care outcomes. Provides a focused physical assessment while being aware of the physical, emotional and spiritual aspects which may influence health, well-being and quality of life. Attends and provides support at the time of death (with physician's order) and per local regulations. Maintains regular communication with the attending physician and RN Case Manager regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required. Monitors and maintains documentation of clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness via-electronic record so that problems, actions, and goals are accurately and clearly stated and changes are reflected as they occur. Qualifications Graduate of an Accredited Practical Nurse or Vocational Nurse Program. Holds current unencumbered license in the state of practice as an Licensed Practical Nurse. Minimum three (3) years' experience as LPN in Community Health, Medical surgical/Acute care setting required. Hospice/home health experience preferred but not required. Must be computer proficient in typing and various programs, including background in EMR. Possess and maintains current CPR certification if required by state. $39.00 - $40.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
