At Healthvision, our approach is grounded in compassion, collaboration, innovation, and dedication - values that shape everything we do.</p>\n<p><span style=\"font-size: 16px\"><br></span></p>\n<p><span style=\"font-weight: bold\">About the Position | Te Mahi</span><br>This is a full-time, permanent role (40 hours per week, Monday to Friday, 8.30 am–5.00 pm), with occasional weekend work as required.</p>\n<p><br>This position is highly autonomous and involves visiting patients in their homes to apply your practical nursing knowledge and assessment skills, case management, and patient education. It will also involve developing positive working relationships with GP practices and other referrers.</p>\n<p><br>The successful candidate will be highly experienced and clinically confident, with a great attitude and excellent wound care knowledge or experience, ideally from a community or district nursing background.</p>\n<p><br>You will be empowered to provide continuity of care for our patients from beginning to end, while we provide great orientation, excellent ongoing clinical support, and access to ongoing education.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">In this role: </span><br>•    You will use your skills to assess and plan Homecare support for our Service Users. <br>•    You will be hands on with service user assessments and Community Support Worker training and competencies. <br>•    You will follow up and review service user goals and progress on a regular basis. <br>•    You will enable service users to live their best lives. <br>•    You will ensure accidents and incidents are reported in a timely manner, and you will ensure any service user concerns are escalated to the Clinical Lead.<br>•    You will work in collaboration with Client Service Managers and the Homecare Clinical Lead to provide outstanding services to our service users in the community. <br>•    You will be competent in your field with the ability to learn different skills and gain a broad knowledge around medication management, enteral feeding, falls and pressure injury prevention and wound care. <br>•    You will work holistically using a client-centred approach and will have a strong local knowledge of our health system.<br>•    You will have the ability to communicate with primary and secondary healthcare, and an understanding of ACC and MOH community contracts is an advantage<br> <br><span style=\"font-weight: bold\">The ideal candidate will have:</span><br>•    Most importantly - a passion for helping service users in the community, full training is on offer!<br>•    Experience working in the community is desirable<br>•    Knowledge of, or experience working with, spinal injury and traumatic brain injury (TBI) protocols is preferred<br>•    Current registration and NZNC practising certificate<br>•    The ability to work autonomously with a strong solution focus<br>•    Strong MS Office skills<br>•    Attention to detail <br>•    Strong and positive communication skills<br>•    Ability to work in a complex and changing environment<br>•    Critical thinking<br>•    Clean full New Zealand Driver’s license<br>•    Must have the legal right to work in NZ</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">We offer:</span><br>•    Competitive Pay Rates {Depending on years of practical experience, the pay for clinical workers can range from $80,000 to $90,000 annually}<br>•    Flexible hours<br>•    Training opportunities<br>•    Supportive team <br>•    Free EAP (Employee Assistance Programme) Services<br></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Hiahia | Interested?</span></p>\n<p>Bring your people skills, initiative, and heart to a role that makes a difference.
