    "ExternalDescriptionStr": "<p>The Patient Family Counselor is responsible for providing counseling and support for patients, families and caregivers. This includes the development of goals and objectives and the direct provision of care for patient, family, and caregiver.</p>\n<p><strong>Responsibilities:&nbsp;</strong></p>\n<ul>\n <li>Provide direct patient care as assigned.</li>\n <li>Communicate findings to physicians, nurses, allied health and extended interdisciplinary team.</li>\n <li>Perform social evaluations including, family dynamics, caregiving abilities, communication patterns, high risk for suicide, neglect or abuse, and plans based on evaluation findings.</li>\n <li>Provide crisis intervention when necessary.</li>\n <li>Educate interdisciplinary group on special needs related to the cultural differences, learning style, learning limitations, communication style, and roles within the family.</li>\n <li>Maintain clinical records on patient, family, and caregiver.</li>\n <li>Assist with advanced directives, and/or transitions to next level of care.</li>\n <li>Serves on various ad hoc and standing committees/meetings as requested.</li>\n</ul>\n<p><strong>Qualifications:&nbsp;</strong></p>\n<ul>\n <li>Master’s Degree Required</li>\n <li>Licensed Social Worker (LCSW) and/or Licensed Mental Health Counselor (LMHC)</li>\n <li>Minimum one of year experience in a hospital or medical setting</li>\n <li>May be a Licensed Marriage/ Family Therapist (LMFT) With more than 2 years experience in hospital/ medical setting</li>\n <li>Knowledge of patient and/or family caregiver support, including the necessary knowledge, experience and clinical skills.</li>\n <li>Analytical ability necessary to solve complex administrative and clinical problems. Ability to evaluate needs of patient populations and develop programs to meet those needs.</li>\n <li>Interpersonal skills necessary to motivate and provide direction.
