Responsibilities include coordinating treatment plans, conducting educational groups, and ensuring continuity of care post-release. The role requires close collaboration with medical, security, and administrative staff to deliver integrated, effective support for inmates undergoing MAT.</p>\n<h2><strong>Key Responsibilities</strong></h2>\n<ul>\n <li>Assists the Director of Recovery and Reentry Services (DRRS) in managing the case management needs of individuals in MAT treatment, ensuring coordinated services integrated with security, medical, and administration.</li>\n <li>Conducts educational groups, provides routine case management services, and ensures appropriate documentation and active involvement in treatment team meetings.</li>\n <li>Provides discharge planning services to support MAT continuation post-release, ensuring all services are documented in the medical record as per protocol.</li>\n <li>Attends clinical supervision, required trainings, and relevant meetings, assists the DRRS with data collection and entry for all MAT visits, and prepares statistics for third-party agencies.</li>\n <li>Networks with administration, correctional officers, medical, and outside providers to maintain positive working relationships, abides by security regulations, protects the confidentiality of inmate information, and adheres to HIPAA Regulations.</li>\n</ul>\n<h2><strong>Qualifications & Requirements</strong></h2>\n<p>Education</p>\n<ul>\n <li>Bachelor’s degree in substance use, social work, psychology, or related field from an accredited college or university or nursing license; LPN</li>\n</ul>\n<p>Experience</p>\n<ul>\n <li>One (1) year experience required.</li>\n <li>Mental Health, Substance Use Disorders, Medication Assisted Treatment, or Case Management in a corrections environment preferred.</li>\n</ul>\n<p>Licenses/Certifications</p>\n<ul>\n <li>Possess and maintains current CPR certification.</li>\n</ul>\n<h2><strong>We are an Equal Employment Opportunity Employer</strong></h2>\n<p>We are committed to fostering, cultivating, and preserving a culture of uniqueness.</p>\n<p> </p>\n<p>We celebrate a variety of backgrounds and are committed to creating an inclusive environment for all employees.</p>\n<p> </p>\n<p><strong>We encourage you to apply!</strong> If you are excited about a role but your experience doesn’t seem to align perfectly with every element of the job description, we encourage you to apply.
