Salary $33.00 / hr - $44.86 / hr Overview Elderwood at Wheatfield is seeking a compassionate and dedicated Full-Time Occupational Therapist to join our growing team of rehabilitation professionals. Position Overview: Occupational Therapy staff members play a vital role in ensuring the health, well-being, and quality of life of our residents. In this position, you will provide comprehensive, individualized therapy services focused on restoring function, promoting independence, and enhancing overall quality of life. The Occupational Therapist is responsible for administering skilled therapy treatments while maintaining the highest standards of care and professionalism at all times. Key Responsibilities: Evaluate residents and develop individualized treatment plans based on their unique needs and goals Provide skilled occupational therapy services in accordance with physician orders and regulatory guidelines Implement therapeutic interventions to improve activities of daily living (ADLs), mobility, strength, and cognitive function Monitor resident progress and adjust treatment plans as needed to ensure optimal outcomes Collaborate with physicians, nursing staff, therapy team members, and families to support coordinated care Maintain accurate, timely, and compliant documentation in accordance with facility policies and state/federal regulations Educate residents and caregivers on therapy techniques and strategies to support continued progress Ensure a safe, clean, and supportive therapy environment Qualifications: Current Occupational Therapy license in New York State required Degree in Occupational Therapy from an accredited program Strong clinical, organizational, and communication skills Ability to work effectively both independently and as part of an interdisciplinary team Prior experience in a skilled nursing facility or rehabilitation setting is preferred, but not required Why Elderwood at Wheatfield? Responsibilities Occupational Therapist: Initiates skilled occupational therapy treatments in accordance with physician’s orders; maintains required contact with physician. Maintains written evaluations, progress notes, MDS records and discharge summaries for individuals in occupational therapy program. Confers with other health care staff regarding special needs of indviduals to enhance rehabilitation or maintenance of function. Provides input for Care Planning meetings and attends during absences of the Director of Occupational Therapy. Maintains statistical records of indviduals’ treatments. Assists with in-service training programs upon request. Records and updates individual’s care plan. Submits requisition for supplies needed in the treatment of individuals to Director of Occupational Therapy. Assumes all duties of the Director of Occupational Therapy, if applicable, during absences including supervision of COTA’s and Occupational Therapy Aides. Assumes responsibility for all necessary documentation. From Up to Qualifications Occupational Therapist: Graduation from an approved school of occupational therapy required. State license to practice occupational therapy required. Two years’ experience in occupational therapy preferred. Interest in occupational therapy treatment programs for the geriatric and subacute populations utilizing a team approach. Ability to direct others necessary. Ability to communicate well verbally and in writing required. Current, valid CPR/BLS credential Sense of responsibility and ability to work well with residents, families, and other staff members required. Qualities of maturity, diplomacy, and ability to work well with others required. Ability to work with geriatric and chronically ill residents, as well as other professionals in a friendly and helpful manner necessary. Ability to perform tasks to established standards of excellence required. This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
