The clinical teams at the area’s clinics are improving outcomes, lowering the number of hospital readmissions and reducing avoidable costs associated with treating chronic conditions. The UnityPoint Clinic team is known for excellence and quality patient care putting patients at the center of their care to provide the best level of health and well-being.</p>\n<p> </p>\n<ul>\n <li>Established team of 2 physicians and 4 Advanced Practitioners</li>\n <li>Diabetes Self-Management program staffed with Certified Diabetes Educators (RN’s and Dietitians) </li>\n <li>Healthy Living Program, a medically managed weight loss program supported by Dietitians and a Health Coach</li>\n <li>Experienced staff, both clinical and non-clinical, who know the patients and the community</li>\n <li>Full-time position, including dedicated administrative time</li>\n <li>24/7 hospitalist program is responsible for adult inpatients</li>\n <li>Fully integrated system with clinics and hospitals on EPIC EMR to improve care coordination</li>\n</ul>\n<h2>Responsibilities</h2>\n<ul>\n <li>Outpatient clinic only; Adult and adolescent patients 15+</li>\n <li>Clinic hours Monday – Friday 7AM-5PM</li>\n <li>No nights or weekends </li>\n <li>Shared call amongst the providers.
