    "requisitionDescription": "<div><p><strong>Job Summary:&nbsp;</strong></p><p>Under the supervision of the Home Care Manager, a CNA-Home Care provides personal care and homemaker services according to individual care plans for participants of the PACE Organization of Rhode Island. Oversight of performance and job duties is conducted by a Nurse during observation visits and more frequently as needed.</p><ul><li>Keeps current on and follows individual participant care plans. Consistently &ldquo;clocks in &amp; out&rdquo; of participants&rsquo; homes and documents completed ADL/IADLs in AxisCare.</li><li>Provides personal care and services as assigned including toileting, bathing/showering, eating assistance, grooming and oral care.&nbsp;</li><li>Provides light homemaking tasks as assigned including meal prep, shopping and laundry services. Ensures a clean, sanitary, and safe environment is maintained.</li><li>Provides behavioral supervision and support for participants, monitors for and responds to behavioral disturbances such as wandering, agitation, restlessness, exit seeking, etc.&nbsp;</li><li>Observes participants for emotional, behavioral, and physical changes in condition including, but not limited to any changes in mood, skin integrity, bowel and bladder status, food/fluid intake, speech, ambulation and transfer ability. Promptly reports participant change of condition to Home Care Department.&nbsp;</li><li>Responds appropriately and in a timely manner to personal alarm devices if present in the home. Adheres to PACE&#39;s Participant Rights Policy at all times.&nbsp;</li><li>Adheres to PACE safety requirements and participant care plans regarding use of assistive devices (e.g., gait belts, hoyer lifts, slide boards, etc.).&nbsp;</li><li>Uses proper universal and standard precautions and COVID-19 PPE protocol according to PACE policy and procedure. &nbsp;&nbsp;</li><li>Assists with implementing recreation programs in a home setting. &nbsp;</li><li>Assists participants with physical activities in support of restorative care plans (e.g. walking for exercise) as assigned and trained by the rehab department to implement in the home setting.&nbsp;</li><li>Notifies Home Care Department of low inventory to avoid running out of supplies.&nbsp;</li><li>Attends staff and participant care planning meetings as requested, and department and agency in-services as required.&nbsp;</li><li>Protects privacy and maintains confidentiality of sensitive participant and agency information.</li><li>Works in PACE-RI Day Centers as assigned.&nbsp;</li><li>Works rotating weekends and/or holidays as assigned. Performs related duties as assigned</li></ul><p><strong>Required Skills &amp; Abilities:&nbsp;</strong><br>Ability to maintain confidentiality<br>Dependable and punctual with ability to maintain consistent attendance<br>Ability to read, write and comprehend English<br>Ability to maintain sound judgement under stress and communicate effectively<br>Ability to proficiently perform clinical/skilled tasks associated within scope of position<br>&nbsp;Proficiency with MS Office Suite or similar software&nbsp;</p><p><strong>Education Requirement:&nbsp;</strong><br>High School Diploma or Equivalent, Required</p><p><strong>Physical Requirements:&nbsp;</strong><br>Physical ability to regularly stoop, bend, kneel, lift, stand, walk, stretch and reach for extended periods of time.<br>Must be able to regularly lift objects up to 25 pounds and at times up to 35 pounds.<br>Must be able to assist a 200-250 lb participant, using proper body mechanics.<br>Must have hearing and vision abilities within normal range (corrected) to drive safely and/or to observe and communicate with participants.<br>Must be able to navigate various departments of the organization&#39;s physical premises as well as related community care settings.<br>Must be able to tolerate exposure to adverse indoor and outdoor environmental conditions (including heat, humidity, dust, smoke, pet contamination, and cleaning supplies), bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards typically associated with a medical or home care setting.&nbsp;</p><p><strong>Experience:&nbsp;</strong><br>1 year of CNA experience, Preferred<br>1 year of experience working with a frail or elderly population, Preferred&nbsp;</p><p><strong>Licensure &amp; Certification Requirements:</strong><br>Rhode Island Certified Nursing Assistant License (CNA)&nbsp;</p><p><strong>Position Requirements:</strong><br>CPR Certification<br>Driver&#39;s License &amp; access to reliable transportation &nbsp;</p></div>\n<br/>Early AM availability strongly preferred | Optional weekend shifts available",
