This includes scheduling intakes, facilitating service linkage, and ensuring individuals are connected to TTC programs in alignment with HHRP/CD3 guidelines and timelines.</p><p>Navigators will maintain accurate and up-to-date documentation in all required systems, track referral outcomes, and collaborate with internal teams and external partners to support housing stabilization and continuity of care.
