Case Manager (Tracy EHP) In-Person Full-Time (40 hours per week) Position in Tracy, CA (Non-Exempt) Who We Are Ruby's Place dba Restorative Pathways (RP) is an innovative nonprofit committed to ending domestic violence, human trafficking, and violent crime through hope, advocacy and connection. Our services include emergency shelter, transitional housing, rental subsidies, mental health services, substance recovery, and wrap-around services. All our services are trauma-sensitive and client-centered and are provided at no cost to our clients. All programs are low barrier. RP is led by survivors with a focus on reflection, innovation, and connection.  We believe in fostering relationships, both in our direct service work and throughout our internal work culture. We aim to cultivate a workplace that appreciates and emphasizes the voices of those most impacted by violence. We recognize these voices are vital to interrupting the cycle of violence and transforming lives towards safety, unity, and trust. You are flexible, creative, and take initiative in handling emergencies. What qualifications are required for this position? You Are Also: Bachelor’s degree in social work, human services, or a related field preferred; equivalent experience may be considered. Minimum of two years of experience providing case management or direct services to individuals experiencing homelessness or housing instability. Experience working in shelter or residential settings is preferred. Knowledge of housing resources, benefits systems, and community services. Strong organizational, communication, and engagement skills. Ability to manage a high caseload in a fast-paced environment. Experience with HMIS or similar data management systems. Knowledge of trauma-informed care, harm reduction, and housing-first approaches. Experience working with individuals with complex needs, including behavioral health or substance use challenges. In possession of a valid CA driver's license, proof of insurance, and be approved by RP insurance as a driver in order to drive agency vehicles.  If a staff member cannot be approved by RP insurance as a driver, employment might not be able to continue.  Instances where staff cannot be approved to drive will be reviewed on a case by case basis to determine if employment can continue in their role without driving agency vehicles.  Items considered include but are not limited to: the shift the staff member holds and the frequency/necessity of driving during that shift, the amount of time before the staff member may be approved to drive by the insurance carrier, the availability of other staff on site to drive agency vehicles as program needs require, etc. Able to complete all required background checks. Additional requirements may be deemed necessary, including fingerprinting (Live Scan), completion of all Community Care Licensing (CCL) documents, and ability to meet CCL licensing requirements. What You Will Do The Case Manager provides direct services to participants residing in a 150-bed congregate shelter, supporting individuals in addressing immediate needs, identifying barriers, and working toward stable and permanent housing. This role is responsible for engaging participants, completing assessments, developing service plans, and coordinating access to housing resources and supportive services. The Case Manager works as part of a multidisciplinary team and collaborates closely with Residential Advocates, the Program Coordinator, and leadership to ensure participants receive coordinated, participant-centered care. This position requires strong engagement skills, the ability to manage a high caseload, and experience working with individuals experiencing homelessness or complex needs. Responsibilities include Participant Engagement and Assessment Engage participants residing in the shelter to assess needs, strengths, and barriers to housing stability. Complete intake assessments and gather relevant information including housing history, income, and service needs. Build and maintain professional, trusting relationships with participants using a trauma-informed approach. Service Planning and Case Management Develop individualized service plans focused on housing stability, self-sufficiency, and access to resources. Provide ongoing case management support through regular meetings, goal tracking, and problem-solving. Support participants in obtaining identification, benefits, employment resources, and other necessary services. Housing Navigation and Coordination Assist participants in identifying and pursuing housing opportunities, including referrals to housing programs and resources. Support participants with housing applications, documentation, and preparation for housing placement. Coordinate with housing navigators, landlords, and community partners to support participants in securing housing. Coordination and Team Collaboration Work closely with Residential Advocates, Program Coordinator and the Facility Manager to identify participant needs and ensure consistent support. Participate in case conferencing and team meetings to coordinate services and discuss participant progress. Collaborate with internal departments and external partners to enhance service delivery. Documentation and Data Management Maintain accurate and timely case notes, service plans, and participant records. Enter and update participant information in HMIS or other required data systems. Ensure documentation meets organizational standards and funder requirements. Participant Advocacy and Support Advocate on behalf of participants to access services, housing opportunities, and community resources. Support participants in navigating complex systems and overcoming barriers. Encourage engagement and participation in services and programming. Complete functional timesheets by assigned due dates in compliance with time allocations. Actively participate in reflective supervision sessions. Create and maintain documentation of processes and procedures related to job duties. Other duties as assigned. Working Conditions This position requires regular on-site presence in a 150-bed congregate shelter. The Case Manager must be able to manage competing priorities, maintain accurate documentation, and engage participants in a dynamic and high-capacity environment. You must be able to stand for long periods of time and perform physical activities, including lifting up to 25 pounds. You will be subject to frequent interruptions and must be able to multitask and handle stress. Local travel may be required. Applying The pay scale for this role is $25 per hour (non-exempt).
