Provides care appropriate to the age of the patients served.  Demonstrates knowledge of the principles of growth and development over the life span.  Assists the registered nurse in assessing, planning, implementing, and evaluating outcomes of care.  Is accountable for own nursing actions and competencies.  Communicates effectively with others.  Performs all duties in accordance with the State of Illinois Nurse Practice Act and established standards, policies, procedures and guidelines.  Adheres to MH Mission and Vision and the ANA Code of Ethics.</p>\n<h2>Qualifications</h2>\n<p><em>Education:</em></p>\n<ul>\n <li>Successful graduation from an accredited practical nursing program</li>\n</ul>\n<p><em>Licensure/Certification/Registry:</em></p>\n<p> </p>\n<p>        Illinois Nurse Practice Act. 225 ILCS 65/55-1 Article 55-10.7.d requirements:</p>\n<ul>\n <li>Official written notification indicating successful passage of the practical nurse licensure examination</li>\n <li>Proof of application for licensure and required licensure fee as a practical nurse</li>\n <li>Proof of submission to a criminal history records check</li>\n <li>Practice as a LPNLP will terminate:</li>\n <li>At three months from the official date of passing the licensure exam (unless official extension granted by IDFPR and documentation of this extension provided)</li>\n <li>Upon receipt of the LPN license from IDFPR</li>\n <li>Upon notification from IDFPR that the application for licensure has been refused</li>\n <li>Upon request by IDFPR that the individual terminate practicing as a LPNLP until an official decision is made by IDFPR to grant or deny a LPN license</li>\n <li>Current BLS/CPR certification</li>\n</ul>\n<p> </p>\n<p><em>Other Knowledge/Skills/Abilities: </em></p>\n<ul>\n <li>Must be able to position and/or transfer an average adult patient and possess gross coordination sufficient to perform other patient care duties as required. Must be able to withstand prolonged periods of standing.  Must be able to perform difficult manipulative skills such as IV insertion and IM injections.</li>\n <li>Environmental working conditions include exposure to blood, bodily fluids and tissue, contagious diseases and/or radioactive or other potentially hazardous materials.</li>\n</ul>\n<p>Annual Requirements:</p>\n<ul>\n <li>Attends all Joint Commission, OSHA, and hospital/nurse manager specified mandatory inservices.</li>\n <li>Validates competencies and obtains credentials as identified in unit specific requirements.</li>\n <li>Completion of annual self-evaluation and submission of peer feedback at time of performance appraisal.</li>\n</ul>\n<h2>Responsibilities</h2>\n<li><strong>Nursing Care</strong></li>\n<ul>\n <li>Data Collection:</li>\n <li>Observes, records and reports objective and subjective patient data. </li>\n <li>Demonstrates ability to detect signs and symptoms of deviation from normal health status. </li>\n <li>Informs RN of abnormal or changing data and of patient/family responses to interventions.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Planning:</li>\n <li>Provides information to assist the RN in developing the nursing plan of care.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Intervention:</li>\n <li>Completely performs tasks/procedures as appropriately delegated (see Table 1). </li>\n <li>Uses equipment safely. </li>\n <li>Takes appropriate action in life-threatening situations/emergencies. </li>\n <li>Accurately administers medications by appropriately delegated routes. </li>\n <li>Utilizes appropriate infection control safety measures. </li>\n <li>Acts to prevent harm or injury to patient, self and others. </li>\n <li>Documents and communicates to RN nursing interventions and patient responses. </li>\n <li>Seeks supervision prior to initiating new or unfamiliar nursing activities and/or with a change in patient condition.</li>\n</ul>\n<p> </p>\n<p><strong><u>Table 1</u></strong></p>\n<p><strong>Per the Illinois Nurse Practice Act, LPNs (and LPNLPs) are restricted from:</strong></p>\n<p><strong><u> </u></strong></p>\n<p>·     Administering medications via central lines</p>\n<p>·     Administering medication via intravenous (IV) push</p>\n<p>·     Administering chemotherapy via IV routes</p>\n<p>·     Starting or adding blood or blood components</p>\n<p>·     Completing and documenting <u>admission</u> comprehensive head-to-toe assessment and <u>initial</u> plan of care</p>\n<p>·     Obtaining verbal or telephone orders from a provider</p>\n<p> </p>\n<ul>\n <li>Evaluation:</li>\n <li>Observes, records and reports patient responses to nursing interventions and other relevant data to the RN. </li>\n <li>Assists the RN to evaluate and modify the plan of care.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Patient/Family Education:</li>\n <li>Assists the RN to identify, implement and evaluate age appropriate teaching interventions.</li>\n <li>Reinforces teaching initiated by other members of the healthcare team.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Communication:</li>\n <li>Informs RN of abnormal and/or changing data and patient/family response to interventions.</li>\n <li>Develops and maintains interviewing skills required for data collection.</li>\n</ul>\n<p> </p>\n<li><strong>Quality and Safety</strong></li>\n<ul>\n <li>Communicates and documents potential risk issues to appropriate personnel.</li>\n <li>Participates in continuous quality improvement.</li>\n <li>Participates in review and revision of policies, procedures, and standards of care.</li>\n <li>Complies with all safety policies.</li>\n</ul>\n<p> </p>\n<li><strong>Accountability</strong></li>\n<ul>\n <li>Advocates for patients and families.</li>\n <li>Appropriately follows the chain of command. .</li>\n <li>Participates in community organizations</li>\n <li>Identifies annual goals for own professional development.</li>\n <li>Provides constructive feedback to strengthen the Division of Nursing and the colleague’s department/unit.</li>\n</ul>\n<p><strong> </strong></p>\n<li><strong>Performs other related work as required or requested.</strong></li>\n<p> </p>\n<p><em> </em></p>\n<p><em>The intent of this job description is to provide a representative summary of the major duties and responsibilities performed by incumbents of this job.
