The Outpatient Counselor documents all provided services in a manner that is compliant with regulatory and third-party payor requirements. She/he works to coordinate and provide treatment services based on the individualized treatment plan including discharge planning. The Outpatient Counselor advocates for and promotes a recovery-based lifestyle.&nbsp;The Outpatient Counselor works closely with the rest of the staff as an integral part of the clinical team. &nbsp;&nbsp;</p><p>&nbsp; &nbsp;</p><p><strong>SPECIFIC DUTIES &amp; RESPONSIBILITIES:</strong></p><ol><li>Administer comprehensive psychosocial assessment tools at intake and at various points in accordance with Odyssey House policy and procedures.&nbsp;</li><li>Provide individual counseling, group counselling and case management services onsite and in the home or community.</li><li>In collaboration with each client, formulates a comprehensive individualized treatment plan, monitors achievement of objectives, reviews and updates the treatment plan in accordance with clinical needs of the client.&nbsp;</li><li>Maintain up-to-date client clinical records including at minimum: treatment planning, group counseling notes, progress notes, and the monitoring of social determinants of health in accordance with regulatory guidelines and program standards. &nbsp;</li></ol><ol start=\"5\" type=\"1\"><li>Provide crisis intervention services.</li><li>Responsible for ongoing client discharge planning beginning at admission.&nbsp;</li><li>Provides ongoing assessment of each assigned client, monitoring for any changes in clinical needs.</li><li>Assures timely an effective care coordination with collateral services providers, family members and criminal/family justice agencies.&nbsp;</li></ol><ol start=\"9\"><li>Delivers clinical services in accordance with evidence-based curricula and training.</li><li>Acts as a liaison with all referral agencies providing regular verbal and written progress reports.</li><li>Establish community and agency affiliations which address the client&rsquo;s need and strengthen the delivery of services.</li><li>Provide education and direct services to family members, as appropriate, regarding client&rsquo;s substance use/misuse and related behavioral issues.</li><li>Conduct collection of samples for toxicology randomly and as needed. &nbsp;</li><li>Attend regularly scheduled staff meetings and case conferences.</li><li>Attend all required in-service training seminars.</li></ol><ol start=\"16\"><li>Participate in quality improvement activities.&nbsp;</li></ol><ol start=\"17\"><li>Other relevant duties as required.&nbsp;</li></ol><h4><strong>REQUIREMENTS: &nbsp; EDUCATION/TRAINING AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES</strong></h4><ol start=\"1\" type=\"1\"><li>CASAC or CASAC-T preferred, or the ability to obtain within 6 months of hire.&nbsp;</li><li>Ability to deliver culturally competent services to a diverse client population in an urban setting.</li></ol><ol start=\"3\"><li>Proficiency with computer operation (Microsoft Word, Excel, Electronic Health Records, and Outlook programs).&nbsp;</li></ol><ol start=\"4\" type=\"1\"><li>Must be able to work a flexible schedule.</li><li>Bilingual Spanish preferred</li></ol><p>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;&nbsp;</p><p><em>Odyssey House is an equal opportunity employer maintaining a non-discriminatory policy on hiring of its personnel.
