We are Hiring! Job Posting: FOSTER CARE CASE WORKER (URM) Location: Rochester, NY Department: CYF Employment Type: Full time Schedule: Monday thru Friday 9am to 5pm Salary: $23.50 - $24.50 (possible more for experience) General Description Under general supervision, monitors substitute family care for unaccompanied refugee minor foster youth, providing counseling and support services in relation to the permanency goal; maintains confidentiality standards as set by New York State statute and defined by NASW. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment. Essential Duties and Responsibilities Formulates treatment plans with involvement of foster parents, DHS, and youth, when appropriate. Maintains home and face-to-face visits scheduled as determined by DHS, State standards and treatment plan to ensure adjustment and progress toward permanency goals. Offers aftercare services as appropriate according to Standards/Case plan and to sustain progress achieved. Coordinates medical, educational, vocational and mental health services. Provides culturally informed casework intervention including use of interpreter, as case requires. Collaborates with other service providers as determined by Standards/Case plan. Attends all court required appointments with client. Participates in ongoing staff training and program development. Works collaboratively with Monroe County Department of Human Services. Meets regularly with supervisor to evaluate and implement professional growth in practice and performance. Informs supervisor in a timely fashion about significant client activities in formal and informal meetings. Initiates professional development through attendance at outside conferences where appropriate; reading and applying current, relevant literature, etc. Participates in required Agency/Department/Program meetings. Completes required record documentation in accordance with Program Policies and Procedures. Maintains client case records. Documents all relevant client interaction in the case record in a timely fashion. Submits required reports and statistics (FASP, permanency reports, etc.) to supervisor according to Department/Program schedule. Facilitates youth’s connection to cultural resources. Explores identified permanency resources, conducts suitability assessment and arranges for visitation where appropriate. Encourages and assists youth in maintaining their own cultural identify while addressing acculturation issues. Is aware of and actively supports the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative, Entrepreneurial, Culturally Competent and Diverse. Provides After Hours Coverage for Lifeline Completes one-week rotation (Tuesday to Tuesday), providing phone coverage from 5pm-9am, as scheduled Responds to Lifeline phone calls within a half hour from Lifeline contact and works with caller to address the immediate crisis, safety plan and come to an appropriate resolution to the call Completes any necessary paperwork as related to Lifeline phone calls Responsible for completing a home visit/hospital visit/transportation as necessary or arranging to have the task completed Other Duties Participates in relevant Agency meetings and/or trainings Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops. Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan.
