Description Wage band range $23.50 - $36.00; however, starting pay is ultimately determined by the scope of the position, the candidate's relevant experience, and internal equity. Bilingual in Spanish is required, and position is eligible to test for a bilingual pay incentive. Under the oversight of the Director of Transitional Youth Services, the Transition Facilitator (TF) is responsible for implementing the clinical case management component of the overall treatment plan. The Transition Facilitator upholds Casa Pacifica's Standards of Behavior and embodies our core values of respect, integrity, compassion, and courage, helping to facilitate a positive living and learning environment. DUTIES AND RESPONSIBILITIES (illustrated by typical activities): Partners with social workers and probation officers to assist in the continued development and progress of transitional- age youth individualized Transitional Independent Living Plan and Agreement (TILP). Transition Facilitator will be able to engage the youth and emerging adults to establish and build rapport through a strength-based approach. The Transition Facilitator will assist the youth and emerging adult in creating an individualized TILP. Transition Facilitator will assist youth in finding their voice to advocate for themselves by clarifying personal rights and role-modeling positive self- advocacy. The Transition Facilitator will also work to help emerging adults develop personal choice and social responsibility. Transition Facilitator will participate in individualized TILP transition conferences for the emerging adults who remain under the supervision of a Social Worker or Probation officer. The Transition Facilitator will assist the emerging adult in addressing their individual needs and goals within Extended Foster Care. Transition Facilitator will provide emerging adult who are parents with “in the moment” suggestions to encourage positive parenting. They will be familiar with community and natural resources to address individual parenting needs such as, respite, child-care subsidy, medical, dental and medical insurance, parenting classes and PCIT. Transition Facilitator support youth and emerging adults in developing a transitional plan that focuses on five life domains based upon the Transition to Independence Process Model (TIP): Living Situation, Employment and Career, Educational Opportunities, Community Life Functioning and Personal Effectiveness and Wellbeing. The goal is to help youth and emerging adults become self-sufficient and develop life skills in the area of life functioning that she/he desires to focus on. Transition Facilitator will be able to directly assist in-care and after-care youth in developing an academic or vocational plan, enrolling in school or vocational training, completing Free Application for Federal Student Aid (FAFSA) applications, linking them with community resources for assistance with coursework, as well as other scholarship applications. The Transition Facilitator will assist the emerging adult in identifying possible employment opportunities, developing a resume, completing job applications, and developing job skills. The TF may do mock interviews, provide on the job coaching, and work with worksites as appropriate. Transition Facilitator will be familiar with or be trained in and expected to use motivational interviewing and collaborative problem-solving techniques to engage youth and assist them as they work toward developing skills in the five life domains. The TF will also be expected to attend other trainings as needed. Transition Facilitator will provide “in the moment” life coaching and create situations that facilitate opportunities for youth to practice life skills. The TF will highlight areas of strengths as well as areas for improvement and encourage youth to explore alternative goals and strategies when necessary. Transition Facilitator will work alongside and with community partners and agencies such as: Human Services Agency, Transitional Housing Program-Plus, Ventura County Behavioral Health Transitions Program, Pacific Clinics TAY Tunnel, Big Brothers/Big Sisters, Kiwanis, Community Colleges, Casa Pacifica Amigos Auxiliary, Casa Pacifica Angels Auxiliary, and the Office of Education-Foster Youth Service and others as needed Transition Facilitator will be responsible for documenting their services and meeting the established client care minute goal of their respective position. Provide Transportation using a personal vehicle as needed. Attends outside training events and conferences as requested. Maintains compliance with all local, County, State, and Federal government regulations to include Title 22, Division 6, and prepares reports required by regulatory agencies. Maintains confidentiality in all matters within the organization with respect to personnel, operations and clients. Must be able to think ahead and address issues before they arise – Preventative system of care. Will participate in a crisis on call rotation for youth and emerging adults participating in services. Must have a professional and team player approach. Perform other duties as required. Qualifications To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. A valid California driver’s license is required for transporting clients. Any changes that may occur to your driving record must be reported immediately to Human Resources and your supervisor. EDUCATION and/or EXPERIENCE : B.A. or B.S. with five years’ experience or an A.A. with seven years working with severely emotionally disturbed young adults. BSW or MSW in social work (or related field) preferred, with 1-3 years of experience in human services, housing services, or vulnerable populations. CREDENTIALS : Certified Social Work Case Manager (C-SWCM) or Certified Advanced Social Work Case Manager (C-ASWCM) is a plus. OTHER SKILLS AND ABILITIES : Ability to use various computers Software, Microsoft Office Suites. Bilingual in Spanish, required. Knowledge of Title 22, Division 6 licensing requirements and knowledge of residential care. Strong organizational, communication and leadership skills. Strong human relations skills and excellent interpersonal skills. Language Skills : Ability to read, analyze and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from clients, regulatory agencies, or members of the business community. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Ability to write comprehensive evaluations and progress notes/ reports on youth’s treatment goals. Mathematical Skills : Ability to add, subtract, multiply and divide in all units of measure. Reasoning Ability : Ability to apply principles of logic or scientific thinking to a wide range of intellectual and practical problems. Ability to deal with nonverbal symbolism in its most difficult phases. Ability to deal with a variety of abstract and concrete variables.
