Use of a personal vehicle is required to complete the essential duties of this position. The Case Manager promotes a strengths-based approach to service that empowers families, supports a community-based philosophy of service, and ensures that all services and community events case managers attend while representing Wellnest are aligned with our mission, vision, and core values.\\n\\n\\t\\t\\t&nbsp;\\n\\n\\t\\t\\tESSENTIAL RESPONSIBILITIES:\\n\\n\\t\\t\\t\\n\\t\\t\\t\\tConducts comprehensive needs assessments with clients and assists them in obtaining needed resources such as health insurance, housing, and low-cost child care through referral and linkage. \\n\\t\\t\\t\\tProvides individual and group rehabilitation services under Department of Mental Health contract or other agency funding.\\n\\t\\t\\t\\tMeets agency&rsquo;s productivity standards for client service hours.\\n\\t\\t\\t\\tCompletes and submits progress notes in a 24-hour timeframe utilizing the agency&rsquo;s electronic health record system and collaborative documentation while serving clients; discusses and receives proof of insurance / Medi-Cal information in the field and submits to the TQM and/or IS Support Team.\\n\\t\\t\\t\\tProvides guidance and assistance to children, parents/caretakers and adults on managing behaviors successfully.\\n\\t\\t\\t\\tProvides guidance and assistance to children, parents/caretakers and adults on managing familial stressors effectively and successfully.\\n\\t\\t\\t\\tAssists clients with identifying internal resources to assist with multiple stressors such as extended family members, grandparents.\\n\\t\\t\\t\\tDevelops and provide client rehabilitation services which assist in improving, maintaining, or restoring daily functioning at home, school, and community in accordance with treatment goals and in collaboration with treatment team (i.e., therapist, psychiatrist).\\n\\t\\t\\t\\tProvides interagency and intra-agency consultation, communication, coordination and referrals.\\n\\t\\t\\t\\tIdentifies and communicates to therapist obstacles preventing clients from connecting with program and/or referral services.\\n\\t\\t\\t\\tTakes initiative to represent Wellnest while supporting the mission by engaging actively in community outreach events, including events related to recruitment of clients and funder activities and, on weekends.\\n\\t\\t\\t\\tAttends community meetings as requested by program director.\\n\\t\\t\\t\\tRepresents the agency at community resource fairs.\\n\\t\\t\\t\\tFollows up with families that drop out of treatment.\\n\\t\\t\\t\\tProvides services in the communities (in client&rsquo;s homes, school or other field-based settings).\\n\\t\\t\\t\\tConducts presentations in the community to help reduce stigma of mental health.\\n\\t\\t\\t\\n\\n\\t\\t\\t&nbsp;\\n\\n\\t\\t\\tREQUIRED QUALIFICATIONS: \\n\\n\\t\\t\\tBachelor&rsquo;s degree in social work, psychology, or other mental health related discipline.&nbsp;Must speak Spanish.
