This individual will serve as a participant advocate, assist with all admission functions, and provide supervision support to the Nurse Case Manager.  This individual will be responsible for the development, implementation, and evaluation of social service care plans for each assigned participant, coordinating services with other agencies/caregivers and continuously evaluating all service and care plans to see that needed services are received. Experience in Dementia setting is highly preferred.",
